Yes. Items can seem to disappear in Fallout 4 if they were moved into storage, cleared when an area reset, picked up by an NPC, shifted or clipped through a surface, or affected by a bug or mod. Check storage and nearby inventories first, then search the item’s last location before trying Bethesda’s troubleshooting steps.
First, identify where the item was
The likely explanation depends on whether you left the item loose in a settlement, put it in a built container, used a pre-existing container, or left it in a world location. Also note whether it was a weapon, whether nearby NPCs could reach it, and whether mods or Creations were active. These details narrow the search, but there is no established universal rule that every item left on settlement ground is permanent.

Why items may appear to disappear
It may have been moved into storage

An area or container may have reset
Players report that items left in some world locations or containers can be cleared when an area resets. This is a risk to consider, not a documented rule for every container: there is no authoritative complete list of which containers reset. An NPC may have taken an accessible weapon
Community reports describe settlers and other NPCs picking up accessible weapons, particularly when ammunition for them is available. A loose object may have moved or clipped through a surface
Physics-enabled clutter can be knocked around. Bethesda’s listing for the Clutter Crafting Creation specifically warns that its crafted items can be knocked over and may clip through surfaces; that warning applies to those objects, not every missing item in the game. A bug, save issue, or mod may be involved

Search the item’s last known position, including around and under it if it was loose clutter.

Travel away and return. If needed, enter an area that triggers a loading screen, then go back to the item’s location.

Exit the game and restart it; if the problem persists, restart the system as well.

Load a save from before the item went missing. Bethesda Support says it cannot restore lost Fallout 4 items or progress, so an earlier save is the documented recovery option. Bethesda Support

Xbox One: additional steps for missing weapons
Bethesda’s Xbox One guidance recommends power cycling the console and testing the issue in a new game. If the problem does not occur in the new game, Bethesda says the existing save is likely corrupt. This is platform-specific advice, not a general diagnosis for every version of Fallout 4.
