That milestone is described as the first Mom kill. Afterward, the community answers say mods no longer prevent progress.
When do mods block unlocks?
Steam community answers identify unlocking the Womb—also referred to as in-game secret number 4—as the point after which you can use mods while earning unlocks. A Steam community participant, Nazemec, summed up the rule as: “Mods prevent nothing past that point.” The comment refers to unlocking the Womb. This is community guidance, not a rule confirmed in official developer documentation. Steam Community: “Do mods disable Steam achievements?” Steam Community: “achievements”
Does the rule apply to every save file?
No: community guidance says the Womb prerequisite must be met separately on each save. Unlocking it on one file does not clear the restriction on another. Steam Community: “Do mods disable achievements in repentance?”
Does it depend on the Isaac version?
Community answers distinguish the editions: they describe Rebirth and Afterbirth as not having the same mod restriction, while Afterbirth+ and Repentance block unlocks before the Womb milestone. Repentance is DLC for The Binding of Isaac: Rebirth, according to its Steam store listing. What about Repentance+ online play?
The official Repentance+ announcement says mods must be disabled and the game restarted for online play. That is a separate online-play requirement; the announcement does not establish whether mods block unlocks in solo play on Repentance+. For that specific version and mode, check current version-specific guidance rather than assuming the solo rule is the same. The Binding of Isaac / Steam Community Announcements

The cited community answers say the restriction ends once the Womb is unlocked on that save. They do not provide an official recovery procedure for every save or version, so they support that milestone rule but not a universal fix for any particular save issue.
