Joel rescues her before the fatal operation, leaving the cure a possibility that is never tested—not a discovery that happens in the story.
What happens to the cure in the game?
Ellie is immune to Cordyceps infection, and the Fireflies believe that studying her could help them develop a cure or vaccine. At the hospital, their doctors plan an operation that would kill her. Joel rescues Ellie before it can go ahead.

Afterward, Joel tells Ellie that the Fireflies had found other immune people, failed to make a cure, and given up. That account is a lie: the game’s official podcast discusses Joel’s deception and the choice he makes to take Ellie away. The cure is never produced, and the story does not show what the Fireflies would have achieved if the operation had happened. Naughty Dog’s official podcast frames the ending around the painful question of whether saving someone you love can justify refusing a possible cure.
Could the Fireflies have made a cure?
For the game’s intended story, Druckmann’s answer is yes. In a 2025 appearance on the Sacred Symbols podcast, as reported by GameSpot, Druckmann said, “Our intent was, ‘yes, they could,’” when asked whether the Fireflies could make a cure. He also acknowledged, “Our science is a little shaky and people are now questioning it.”

That clarification establishes creator intent, not scientific certainty. The game never demonstrates that the procedure would work, and Druckmann’s caveat makes clear that the fictional premise is open to questions. The most precise answer is that the Fireflies were intended to have a real chance at making a cure, but Joel’s intervention means the story never confirms the outcome.
Was Joel right to stop them?
The game does not supply an objective moral verdict. Joel prevents a possible cure while saving Ellie from a procedure that would kill her, and she has not agreed to pay that cost. The Fireflies, meanwhile, are willing to sacrifice a child in pursuit of a cure that remains uncertain within the events of the story.

Druckmann’s earlier commentary on the ending emphasizes its emotional rather than purely medical question: “It’s like there is no cure. It’s not about that. It’s about saving the ones we love.” That thematic framing does not cancel his later statement that the creators intended the Fireflies could succeed. It helps explain why the ending can support opposing readings: Joel may be seen as denying humanity a possible cure, while the Fireflies may be seen as wrong to kill Ellie for one.

The HBO series preserves the central hospital dilemma: Ellie is immune, the Fireflies plan a procedure that would kill her, and Joel rescues her. The adaptation is not identical to the game, however. The infection mechanics differ, including changes involving spores and tendrils. Naughty Dog describes the game and show as distinct experiences built around the same relationship in its comparison of the game and adaptation.

So, for both the original game and the first-season HBO story, no cure is actually found. Druckmann’s 2025 comment specifically clarifies the intended possibility in the game’s ending; it should not be treated as proof that a cure is made in the show or that later adaptation installments must resolve the question the same way.
