No—Mia does not die in the continuing, canonical story. However, Capcom’s official timeline places Mia among the main characters of Resident Evil Village, set three years after the Baker House Incident, and describes Ethan living with his family before that game begins.
So the precise answer is: your choice can affect what happens immediately in your version of RE7, but Mia survives in the story that continues into Village.

The short answer: Mia survives the story that continues into Village
Mia Winters is alive in the canonical continuity after Resident Evil 7. Capcom’s official franchise timeline lists Mia as one of the principal characters in Resident Evil Village, alongside Ethan, Chris and Rose. The timeline dates Village to February 2021, three years after the Baker House Incident, and says Ethan is living peacefully with his family before the events of that game. That family context necessarily includes Mia.
This does not mean every player sees exactly the same immediate result at the end of RE7. The game presents Ethan with a difficult choice, and Capcom’s launch material specifically warns that players will face hard decisions. The choice changes the short-term outcome shown in an individual playthrough. It does not erase Mia from the continuing storyline represented by Village.

Capcom’s official timeline is the strongest evidence for the canon answer: Mia appears in both games’ character listings, and the later game occurs years after the events in Louisiana. You can read the timeline at Capcom’s Resident Evil universe history.
Why the ending causes confusion
Mia is introduced as Ethan Winters’ missing wife. Capcom’s description of Resident Evil 7 has Ethan searching for her at a Louisiana plantation, making her disappearance the central mystery that drives the opening of the game. Capcom’s Gold Edition announcement likewise describes Ethan’s search for his missing wife and identifies the game’s PlayStation 4, Xbox One and PC releases: Capcom’s Gold Edition announcement.
Because the player is trying to find Mia, scenes involving her naturally feel like a final judgment on whether she lives or dies. The game then asks Ethan to make a consequential choice. A player who sees one immediate outcome may reasonably assume that outcome is the entire series continuity, while another player may see a different result. The distinction between “what happened in this playthrough” and “what the next game treats as history” resolves the apparent contradiction.

The official chronology gives several facts that answer the question without requiring speculation about every ending scene:

- Resident Evil Village takes place in February 2021.
- Village is set three years after the Baker House Incident. That incident is the name the timeline uses for the events associated with RE7.
- Ethan is living peacefully with his family before the next crisis. Mia is identified as a main character of that later story, and Rose is part of the family context described before her kidnapping.
Those points establish continuity across the two games. Whatever variation a player encounters at the end of RE7, the later canonical narrative proceeds with Mia as a living character connected to Ethan and Rose.
The player-choice caveat
Resident Evil 7 does not present its finale as a completely fixed sequence for every player. Ethan must choose between two people, and that decision affects the immediate resolution. Capcom’s release announcement calls attention to the game’s “hard decisions,” supporting the fact that the player’s action matters: Capcom’s launch announcement.

Chronology: where Mia fits in the series
Putting the games in order makes the answer easier to follow.

This chronology is why the answer should not be reduced to a single screenshot or one player’s ending. The series itself supplies a later point of reference, and that reference places Mia in the post-RE7 timeline.
What “canonical” means here
“Canonical” means the version of events the continuing series treats as its official history. Games sometimes let players make choices for different endings, bonus scenes or replay value, while a sequel follows one established path. In this case, Village is the relevant follow-up. Its setting is explicitly after the Baker House Incident, and its character list includes Mia.

- The official timeline supports Mia’s survival into Resident Evil Village.
- Capcom’s game descriptions support Ethan’s search for Mia and identify her as central to RE7.
- Capcom’s launch wording supports the presence of difficult player decisions.
- A pre-release PlayStation Blog preview discusses an early found-footage segment labeled “Mia” and the mystery surrounding whether the young woman will escape. Because that article predates the game’s release, it is context for the setup, not proof of the final ending. It is available at the PlayStation Blog preview.
None of those sources requires inventing a quotation or filling in an unverified scene. The later timeline is enough to answer the survival question, while the launch material explains why the immediate ending can vary.

If you want to preserve the intended tension, make the decision without looking up a branch-specific walkthrough. The game is designed for the choice to feel difficult, and the immediate result belongs to the version of the story you create. After finishing, you can use the official timeline to place that ending in the broader series continuity.

“The choice has no effect at all.”
That is also too broad. Capcom explicitly presents difficult decisions, and the player’s choice affects what happens immediately. The choice matters within the playthrough even though the later canon establishes Mia’s survival.
“A preview article proves the final ending.”
The PlayStation Blog material was published before release and describes the setup around a found-footage segment. It should not be treated as a complete account of the finished game’s ending.
“Mia is only important in RE7.”
Capcom’s timeline lists her among the main characters of both Resident Evil 7 and Resident Evil Village. Her role therefore extends beyond the opening mystery in Louisiana.
