Search the garrison before leaving: the quest reference says this area cannot be revisited. Finish the quest before the major city choice, which PC Gamer reports can cut it off.
How to start A Slow Poison
After completing Sea of Blood, look for a quest note in the Lighthouse Library. PowerPyx says the quest is available after Sea of Blood and recruiting Lucanis; Gamer Guides specifies that The Smuggled Relic Case must also be complete before the note appears. Because the listed prerequisites differ, complete the relevant Lighthouse and main-story conversations, and check again after The Smuggled Relic Case if the note is missing. (PowerPyx; Gamer Guides)
Read the note, then travel to Treviso and speak with Teia and Viago at the Cantori Diamond. They send Rook to investigate the Antaam’s poison operation and their missing scout, Dareth.
Rescue Jacobus in the Drowned District
- Go to the warehouse by the water in the Drowned District.
- Defeat the Antaam there. Game8 identifies one enemy as Taarathaa the Slaver.
- Free Jacobus from the cage. He tells Rook that Dareth, his cousin, was killed while going to retrieve a contract.
Follow the crow marks to Dareth’s contract
Follow the purple crow graffiti through the Drowned District to Dareth’s dead drop. The contract is on a wooden table in a rooftop room, accessible through a broken second-story window. It names the qamekmaster as the target: the Antaam operative making poison for the Butcher’s forces. It also directs the Crows to identify and eliminate the traitor if the evidence reveals who it is.

The Dragon Age Wiki describes three crow-mark locations in the district, but says interacting with each mark is not required if the quest marker already leads to the destination. (Dragon Age Wiki; Game8)
Reach the Antaam garrison and defeat the qamekmaster
Use the rooftops and ziplines to approach the garrison, fighting Antaam soldiers along the way. Enter through the attic and proceed down through the building until you confront the qamekmaster.

After the fight, inspect the page titled “Journal Page in a Shaky Hand.” It indicates that a human helped the Butcher occupy Treviso and that someone stole some qamek. The quest reference does not identify the thief, so the journal does not provide a confirmed identity to pursue.
Loot the garrison before you leave
Explore the side rooms and chests before exiting. The Dragon Age Wiki says the garrison cannot be revisited after this quest. Reported contents vary by guide and source wording:

Return to Teia and Viago
Once you have finished exploring, go back to the Cantori Diamond in Treviso and report to Teia and Viago. Game8 says the final dialogue choices are interaction-only and do not change quest progression.

Rewards and quest timing
Game8 and Gamer Guides report a reward of 750 XP/EXP and +250 Antivan Crows Strength. Game8 also describes an increased bond with current companions; Gamer Guides additionally lists the Contoured Skirmish Jacket. Treat guide-specific item descriptions as reported rewards rather than a promise that every listed item is awarded to every character. (Game8; Gamer Guides)
PC Gamer reported in 2024 that A Slow Poison may be cut off by the first major city decision and recommends completing it before A Warden’s Best Friend. Lockout behavior may depend on the selected path and game version; for a completion-minded playthrough, do the quest before making the major city choice rather than waiting until afterward.
