NFL.com’s Week 4 quarterback index puts Brock Purdy, Patrick Mahomes and Dak Prescott in its top three, while NFL Fantasy’s start/sit advice makes Tyler Shough its Start of the Week. Those are different kinds of guidance, not a single agreed-upon fantasy ranking. For a Week 4 lineup call, weigh recent production, rushing value, matchup, league scoring and confirmed availability.
Updated October 3, 2026. Weekly rankings and injury statuses can change; check the official status and kickoff time for each player before locking your lineup.
Week 4 quarterback rankings: the top tier
NFL.com’s QB Index, published September 30, 2026, ranks quarterbacks as football players rather than projecting fantasy points for a particular scoring format. Its top three are:
- Brock Purdy — No. 1
- Patrick Mahomes — No. 2
- Dak Prescott — No. 3
The index says its arrows show movement compared with the Week 3 index; movement is not a fantasy-points projection. NFL.com’s season-to-date figures after three games offer context for those placements:

These are NFL.com’s reported statistics through three games, not projected Week 4 fantasy totals. Purdy leads this group in passing touchdowns and rushing yards; Mahomes has the most passing yards. League scoring determines how much those differences matter.
What NFL Fantasy recommends starting or sitting
NFL Fantasy’s Week 4 Start ’Em, Sit ’Em article is more directly aimed at fantasy lineup decisions, but its picks are recommendations rather than guaranteed outcomes. It names Tyler Shough its Start of the Week and says he scored at least 22 fantasy points in each of his first three games.

Sit recommendation
NFL Fantasy names Justin Herbert its Sit of the Week. Treat that as a source-specific caution, not a universal rule: roster alternatives, scoring and confirmed game-week circumstances can change the decision.
Why the rankings differ
A quarterback index, a start/sit column and a projection set answer different questions. NFL.com’s index ranks quarterbacks; NFL Fantasy gives roster advice. NBC Sports’ October 1, 2026, Week 4 QB rankings provides an additional editorial cross-check: its description highlights Purdy despite receiver issues, Jordan Love in a favorable road matchup at Tampa Bay, and Shough as a player looking to establish himself in the top 12. That description is not a full ranking list, so it does not establish exact placements for those players.

- Recent production and role: Consider passing volume, efficiency, touchdowns and interceptions, along with whether the quarterback is handling a stable share of the offense. The three-game stat lines above are context, not a forecast.
- Rushing contribution: Rushing attempts and yards can add points and raise a quarterback’s floor, depending on how your league scores them.
- Opponent and game environment: Compare matchup quality, likely game script and opponent fantasy points allowed. Pro-Football-Reference’s Week 4 matchup resource says its matchup ranks use playing percentages, fantasy points and opponent fantasy points allowed.
- Availability and timing: Confirm practice participation, final game status and kickoff timing before deciding. The official NFL injury report excerpt available for this week includes entries dated September 17 and 20; it does not establish every quarterback’s final Week 4 status. The NFL Week 4 schedule is the place to check game timing.
For a close choice, a favorable matchup or meaningful rushing role may outweigh a small difference in a general ranking. Do not treat an early-week recommendation as confirmation that a player is active.

Standard, half-PPR and PPR scoring usually affect quarterback comparisons less than passing- and rushing-specific settings, but league rules still matter. Check points for passing touchdowns, passing yards, interceptions, rushing touchdowns and rushing yards. In superflex or two-quarterback formats, positional scarcity and the need to start an additional quarterback can also make a lower-ranked starter more valuable to your lineup than a typical one-quarterback ranking suggests.
