If the part of I Am Plague that interests you most is building a dangerous plague-themed force, start with Plague Inc.: Evolved for disease-spread strategy, Omen Exitio: Plague for branching Lovecraftian investigation, or Thymesia for direct action combat in a plague-mutated world. They share the subject matter, but not the way you play: one is a global simulation, one is interactive fiction, and one is a hands-on action RPG.

I Am Plague itself is not released yet. Greedy Games and Digital Vortex Entertainment describe it as a dark incremental roguelite for PC, with automatic combat, permanent progression, mutation choices, and a rat horde fighting through a 200-plus-floor underground fortress. Steam currently lists a 2027 release window, while the publisher announced an open Steam playtest in October 2026. Both dates can change, so check the current Steam listing before planning around them.

What makes a game “like I Am Plague”?

I Am Plague is an unusual reference point because its strongest identity comes from a combination of systems rather than a single genre label. You lead a rat horde instead of an individual hero, combat is automatic rather than manually controlled, and each run feeds permanent growth. Mutation choices can be combined into synergies, so the central question is how to assemble a stronger build before the next death. The setting is a dark underground fortress with more than 200 floors.

That means a similar game can match one part of the formula without matching the others. A disease simulator may reproduce the strategic pleasure of spreading and adapting a pathogen but have no battles. A narrative adventure may deliver plague atmosphere and difficult choices but no build crafting. An action RPG may put you in a plague-ravaged kingdom but require precise attacks instead of letting a horde fight automatically.

#1 Best Overall
A Plague Tale: Requiem PS5
  • Sequel to the award-winning adventure, A Plague Tale: Innocence
  • A spectacular grounded tale twisted by supernatural forces
  • Use a variety of tools, sneak, fight, or unleash hell and rats
  • Stunning visuals combine with an enthralling score
  • Includes The Protector Pack DLC: Special crossbow skin, exclusive cosmetics for Amicia, and bonus crafting material

The most useful way to compare the alternatives is to ask four questions:

  • Control model: Are you directing automatic combat, managing a strategic simulation, selecting narrative choices, or controlling attacks directly?
  • Your relationship to plague: Are you creating a disease, carrying it, investigating it, or trying to survive its consequences?
  • Progression: Does power come from run-based mutation synergies, a broader campaign structure, or story decisions?
  • Tone: Is the experience a systems-driven simulation, Lovecraftian mystery, or dark fantasy action game?

At-a-glance recommendations

Game Best match for Control model Role in the plague Progression focus Tone
Plague Inc.: Evolved Disease-spread strategy Strategic simulation Creator and optimizer of a pathogen Adapting a disease as resistance and world conditions change Systems-driven simulation
Omen Exitio: Plague Plague-centered narrative choices Interactive fiction Investigator confronting an outbreak and its secrets Branching decisions and story outcomes Lovecraftian mystery
Thymesia Direct combat in a plague setting Action RPG Survivor fighting plague-created threats Character and combat development through action Dark fantasy

Choose Plague Inc.: Evolved if you want the closest substitute for optimizing a spreading disease. Choose Omen Exitio: Plague if atmosphere, investigation, and consequences matter more than combat systems. Choose Thymesia if you want to physically fight through a plague disaster and would rather control every attack yourself.

Plague Inc.: Evolved — the best fit for disease-spread strategy

Plague Inc.: Evolved is the clearest recommendation for the strategic side of I Am Plague. Its core premise is that you evolve a disease and attempt to infect the world. You are not steering a rat swarm through rooms, but you are still making a sequence of high-impact decisions about how a pathogen develops while conditions become more difficult.

Why it matches

The overlap is the pleasure of building a system that becomes more dangerous over time. In I Am Plague, that means choosing mutations and combining their effects into synergies for an automatically fighting horde. In Plague Inc.: Evolved, it means shaping a pathogen to spread and evolve as the world responds. Both ask you to think several steps ahead rather than simply react to the next enemy.

It is also the strongest choice if the phrase “plague-themed” means an actual disease-management challenge to you. The disease is the thing you develop, and the global population is the problem your strategy must overcome. That gives the game a broad, map-level perspective rather than the fortress-by-fortress progression of I Am Plague.

How the experience differs

Plague Inc.: Evolved is a strategic simulation, not an incremental roguelite with automatic battles. There is no rat horde to watch fight through a 200-plus-floor underground fortress, and the main activity is not selecting combat mutations between encounters. Your decisions operate at the scale of a worldwide disease plan.

Rank #2

The relationship to failure is different as well. I Am Plague makes death part of a permanent-progression loop: a run ends, but the horde can become stronger for the next attempt. Plague Inc.: Evolved instead emphasizes whether your disease plan can adapt to changing conditions and resistance during its scenario. If you want a game where the pathogen itself is the build, this difference is a benefit; if you specifically want repeated dungeon runs, it will feel more like a strategic alternative than a replacement.

Who should start here

  • Players who enjoyed the idea of manipulating infection more than the underground setting.
  • Players who like planning an evolving system against escalating resistance.
  • Players who prefer deliberate strategic decisions to automatic combat watching.
  • Players who want the plague to be the primary mechanic, not just the atmosphere.

Start here when your favorite imagined part of I Am Plague is “make the disease stronger, find a synergy, and overcome the response.” Do not choose it expecting a horde-management roguelite or direct action.

Omen Exitio: Plague — the best fit for narrative and investigation

Omen Exitio: Plague takes the plague theme in a very different direction. It is a Lovecraftian interactive-fiction adventure inspired by the choose-your-own-adventure gamebooks of the 1980s and 1990s. The emphasis is on investigation, atmosphere, and branching decisions rather than optimizing a combat build.

Why it matches

This is the right recommendation when what drew you to I Am Plague was the ominous premise and the feeling that a spreading catastrophe should have consequences beyond a health bar. The plague is part of a mystery to confront, and your decisions determine how the story develops. The tension comes from choosing what to investigate, whom to trust, and which risk to accept, not from finding the most efficient mutation combination.

The Lovecraftian framing also makes it a strong tonal companion. Both games can appeal to players who want bleak, unsettling worlds rather than a light or purely comedic treatment of disease. Omen Exitio: Plague is more concerned with dread and discovery, while I Am Plague turns its darkness into a systems loop built around a rat horde.

How the experience differs

There is no automatic battle loop to master and no permanent progression system comparable to I Am Plague’s mutation-based growth. Its structure is interactive fiction: you read, investigate, and make choices that can send the story down different branches. The most important “build” is the path created by your decisions.

Maximum Entertainment 34 Sports Games - World Edition (PS5)
  • Enter the stadium and take part of one of the biggest competitions across 34 sports: from swimming, soccer, extreme bike, weightlifting, trampoline and much more!
  • Defend your own country and visit the World: United States, Australia, Japan, France…
  • Train yourself alone or compete with your friends!
  • An enhanced version of “30 Sport Games in 1”, with revamped design, new games and more customisation!

That makes it a poor substitute if your goal is to experiment with combinations, push through increasingly difficult floors, or watch a force grow stronger after each death. It is a good substitute if you want the plague to create moral and investigative pressure. Expect a story-led pace rather than a combat-led one.

Who should start here

  • Players who want plague fiction with mystery and historical choose-your-own-adventure influences.
  • Players who value branching decisions and consequences over mechanical optimization.
  • Players who prefer Lovecraftian atmosphere to a systems-heavy simulation.
  • Players who are happy to investigate an outbreak rather than create or command one.

Choose Omen Exitio: Plague when “like I Am Plague” means dark plague fiction and a compelling premise. Choose something else when the essential feature is a repeatable combat-and-progression loop.

Thymesia — the best fit for direct plague combat

Thymesia is the choice for players who want to fight in a plague setting with their own hands. It is an action RPG set in Hermes, a kingdom where an attempt to weaponize and contain a plague leads to mutation and hostile creatures. The setting supplies the plague connection; the gameplay supplies direct, character-controlled combat.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Why it matches

The common ground is a world transformed by disease and mutation. The plague is not an abstract background detail: it has been weaponized, containment has failed, and the result is a kingdom filled with mutated threats. That gives Thymesia a more immediate survival perspective than a global disease simulator.

It also provides the strongest physical sense of danger among these recommendations. Instead of watching an automated horde resolve encounters, you control the action and respond to hostile creatures yourself. For someone who likes the grim setting of I Am Plague but wants timing, positioning, and hands-on fighting, this is the most natural pivot.

How the experience differs

Thymesia is not an incremental roguelite about a rat army. Its progression is tied to an action-RPG structure rather than a run ending with permanent horde mutations. You will not be directing an automatically fighting swarm through a 200-plus-floor fortress, and the plague is something the protagonist must survive and understand rather than a pathogen you deliberately spread.

The control difference is decisive. I Am Plague asks you to make build choices and let automatic combat express them. Thymesia asks you to execute the combat yourself. That makes it more demanding moment to moment, but it also gives every encounter a personal sense of risk that an automatic system cannot reproduce.

Who should start here

  • Players who want a plague-mutated setting with direct action combat.
  • Players who prefer controlling a single fighter to managing an automatic horde.
  • Players interested in plague alchemy, weaponization, and hostile mutations as world-building elements.
  • Players who want dark fantasy action rather than disease simulation or interactive fiction.

Pick Thymesia when combat control matters more than matching I Am Plague’s progression model. It is the best “same mood, different mechanics” option in this group.

Which game should you play first?

Choose by the mechanic you want to preserve

If you want to preserve… Play… Reason
Pathogen optimization and adaptation Plague Inc.: Evolved You evolve a disease and manage its spread against changing resistance.
Plague atmosphere, mystery, and difficult choices Omen Exitio: Plague The experience is built around Lovecraftian interactive fiction and branching decisions.
Mutated creatures and hands-on fighting Thymesia You directly control an action-RPG protagonist in plague-ravaged Hermes.

Choose by the role you want to play

  • Creator: Select Plague Inc.: Evolved if you want to design and evolve the threat.
  • Investigator: Select Omen Exitio: Plague if you want to uncover what the outbreak means and decide how to respond.
  • Survivor: Select Thymesia if you want to fight through the consequences of a failed attempt to control plague.
  • Horde leader: Wait for I Am Plague if automatic combat, rat-horde management, mutation synergies, and permanent progression are the features you specifically want together.

What to know before waiting for I Am Plague

The most important practical fact is that I Am Plague is still upcoming. The current Steam listing gives a 2027 release window, not a fixed launch date. The publisher also announced an open Steam playtest beginning in October 2026. A playtest is not the same thing as a finished release: features, balance, progression, and availability can change while development continues.

Because the release window and playtest status are time-sensitive, verify the current Steam page and the publisher’s latest announcement before assuming the game is available, before planning a purchase, or before judging the final feature set. The stable description at present is the design promise: a dark incremental roguelite in which automatic combat carries a rat horde through a 200-plus-floor underground fortress, mutation choices create synergies, and deaths contribute to permanent progression.

Bottom line: the closest match depends on what you mean by “like”

There is no one-to-one replacement for I Am Plague in this shortlist because its combination of automatic rat-horde combat, mutation synergies, permanent progression, and underground-fortress structure is distinctive. Plague Inc.: Evolved is the best next game for strategic disease manipulation. Omen Exitio: Plague is the best for plague-centered narrative and Lovecraftian investigation. Thymesia is the best for direct combat against plague-created horrors.

Use the first game to test the strategic idea, the second to explore the fiction and atmosphere, and the third to experience the action side. When I Am Plague reaches its planned release, the key question will be whether its automatic combat and permanent mutation loop deliver the same depth promised by its premise.
