The most convincing open worlds are not defined by a single feature. Some stand out because NPCs follow routines; others because they react to what the player does, or because their roles and relationships make a place feel socially grounded. Kingdom Come: Deliverance II, the original Kingdom Come: Deliverance, and Fable illustrate these different strengths—but the available evidence does not establish one universal winner.
What makes an open world feel lived in?
“Lived in” can describe several distinct things. A character might follow a day-and-night schedule without noticing the player, respond to the player’s conduct, or simply have a name and social role that make their presence meaningful. A game can excel at one of these while offering less evidence of another.

- Routine depth: Do people move between work, meals, home, and leisure, or mostly remain in place?
- Reactivity: Do they respond to the player, reputation, or unexpected events?
- Persistence: Does activity continue when the player is away, or is detailed simulation concentrated around the player?
- Population and density: How many people are represented, and where are they concentrated? A large count alone does not make NPCs believable.
- Social and historical grounding: Do people’s occupations, traits, relationships, and setting help explain their behavior?
These are practical ways to compare world design, not a standardized industry rating or a cross-game benchmark.
Games to consider, by the kind of believability you want
Kingdom Come: Deliverance II: routines at a large scale
Warhorse Studios’ GDC session description says NPCs in the first game follow daily routines and react to player actions even after the player has left. For Kingdom Come: Deliverance II, the session describes a map with nearly 2,400 NPCs, around half of them concentrated in one city. Those figures come from Warhorse’s presentation, not an independent census, and they do not mean every character receives equal simulation detail. The same presentation discusses adjusting AI simulation detail to manage frame time and memory. (Warhorse’s GDC session)
#1 Best Overall
The game’s official page describes an open-world medieval setting that includes taverns, forests, and castles. That supports the setting’s variety; the NPC simulation details come from Warhorse’s GDC presentation. (Deep Silver’s official game page)
Kingdom Come: Deliverance: routines affected by the player
CRYENGINE’s official showcase for the original Kingdom Come: Deliverance describes every character as having a daily routine that can be affected by the player. It also describes a world of forests, a sprawling city, and dozens of villages and hamlets. These are claims from the game’s official presentation, rather than an independent evaluation. Fable: identifiable people and social response
In an Xbox Wire interview, Fable general manager and game director Ralph Fulton describes NPCs as having names, roles, and personality traits. Their opinions can include liking, disliking, fearing, or being attracted to the player, and those opinions can reflect reputations the player has earned. This is a different kind of world detail from a daily schedule: people have legible identities and can respond socially to the player. (Xbox Wire interview with Ralph Fulton)

A schedule can make a world seem active, but its timing and geography have to work together.

Why a crowded world is not necessarily a fully simulated one
NPCs can appear to live independent lives without every person being simulated in full detail at every moment. Warhorse’s GDC session describes AI level-of-detail techniques intended to keep frame times and memory reasonably low while supporting a large population. In practice, a game may allocate more simulation detail to nearby characters and less to people outside the player’s immediate area.
A 2014 academic paper on AI for large open virtual worlds discusses the cost of authoring many behaviors, the need for tools and debugging support, and the challenge of making animation, speech, and behavior transition smoothly. It also compares approaches that simulate nearby characters with more continuous world simulation. Its discussion of particular games reflects the authors’ analysis at the time of publication, not a current, comprehensive ranking. (2014 paper on AI for large open virtual worlds)

Warhorse lead scripter Martin Ziegler said in an Epic Games Store interview: “I’m really happy with how we portray Kuttenberg in Kingdom Come: Deliverance II, especially with how deeply researched it is—and consequently, how it feels like a real, lived-in city.” That is his description of the city, not a comparative rating.
