Hamstring Injury Recovery: When to Rest, See a Doctor, and Start Rehab

Rest from aggravating activity at first, use NHS-recommended PRICE care for a mild to moderate injury, and seek advice for severe or worsening symptoms. Recovery and return to sport depend on injury severity and function.

A mild hamstring strain may improve in a few days, while a severe muscle or tendon injury can take weeks or months. Protect the leg and reduce aggravating activity at first, but do not keep it completely still indefinitely. Get medical advice for severe or worsening symptoms, trouble standing or walking, or significant swelling or bruising. Remote advice cannot determine how badly the muscle or tendon is injured.

What a hamstring injury can feel like

A hamstring injury can cause sudden sharp pain at the back of the thigh, sometimes with a popping sensation. Tenderness, swelling, bruising, stiffness, or difficulty standing and walking may follow. These symptoms can occur with injuries of different severity, so they are not enough to grade an injury at home.

A severe injury can involve a tendon pulling away from bone, rather than a strain limited to the muscle. A pop, pronounced bruising, inability to use the leg, or pain high near the sit bone deserves careful clinical assessment. Clinicians may use an examination and imaging such as an X-ray or MRI to assess severity or check for tendon avulsion; do not try to diagnose that yourself. What to do in the first two to three days

For a mild to moderate injury, NHS guidance recommends PRICE during the first two to three days. The aim is to protect the leg and limit aggravation while symptoms settle.

  1. Protect: Avoid movements or activities that could injure the leg further.
  2. Rest: Stop sport and reduce activity that aggravates the pain.
  3. Ice: Apply an ice pack or frozen item wrapped in cloth for up to 20 minutes every two to three hours. Never put ice directly on the skin.
  4. Compression: If comfortable, use a tubular or elastic bandage that is snug but not tight. Remove it before sleep.
  5. Elevation: Raise the leg when you can.

Should you rest or walk on a hamstring injury?

Rest initially means stopping sport and protecting the leg from movements that worsen the injury; it does not mean committing to prolonged bed rest. When pain no longer prevents movement, try gentle movement and increase activity according to symptoms. The NHS advises: “When you can move your leg without pain stopping you, try to keep moving it so your hamstring does not become stiff.”

Do not force walking through pain, and do not jump straight back into strenuous exercise. Too much avoidance can contribute to muscle shrinkage and scar tissue, while strenuous activity too soon can worsen the injury. The right balance is gradual movement that your symptoms allow.

When to seek medical advice

In England, contact NHS 111 if:

  • The pain is very bad or getting worse.
  • Swelling or bruising is substantial or increasing.
  • It hurts to stand or walk on the affected leg.
  • The leg is very stiff or difficult to move.
  • Self-care is not helping.
  • You have a high temperature or hot-and-cold shivers.

This NHS 111 route is specific to England. If you are elsewhere, use your local urgent medical advice service for the same warning signs. Seek careful assessment for a pop with pronounced bruising, inability to use the leg, or pain high near the sit bone, as these can accompany a more serious tendon injury. The clinician—not a remote guide—can decide whether examination or imaging is appropriate.

How to start rehabilitation and return to sport

As early pain and swelling settle, rehabilitation can progress from gentle stretching to activities such as walking or cycling and then strengthening. A physiotherapist can tailor the plan, particularly for severe, recurrent, or slow-to-heal injuries. AAOS describes physical therapy as restoring range of motion and strength, starting with flexibility and gradually adding strengthening once initial pain and swelling have settled. NHS and Northern Ireland public guidance likewise support a gradual return to movement; access to physiotherapy varies.

For sport, recovery should build toward the demands of the activity rather than follow a fixed timetable. The 2022 clinical practice guideline for hamstring strain injury in athletes and an international consensus identify exercise choice and dosage, progression criteria, running and sprinting, and return-to-sport criteria as important rehabilitation elements. Imaging may help classify an injury, but the consensus says it should not be used to decide whether an athlete is ready to return. A clinician or physiotherapist can help assess functional readiness and guide progression.

How long does a pulled hamstring take to heal?

There is no reliable universal recovery promise: timing depends on the injury’s severity, whether a tendon is involved, and the person. NHS guidance says a mild hamstring injury often improves in a few days, while a severe one may take weeks or months. AAOS says a complete grade 3 muscle tear can take months. After proximal tendon repair, rehabilitation typically takes at least six months; after distal repair, AAOS gives about three months before athletic activity. These are different injuries and treatment situations, not interchangeable forecasts for an individual.

A 2018 peer-reviewed review reports that nearly one third of hamstring strains recur within the first year after return to sport. That is a reported recurrence estimate across the reviewed evidence, not an individual’s personal probability. It is a reason to take progressive rehabilitation and return criteria seriously rather than rushing back.
