Symptoms can suggest how serious an injury is, but they cannot confirm its grade or give a reliable return-to-sport date. Marked weakness, extensive bruising, or difficulty walking calls for prompt medical assessment.
What is the difference between a hamstring strain and a tear?
“Hamstring strain” is a broad term for an injury to the muscles or tendons at the back of the thigh. It may mean the tissue has been stretched or pulled, partially torn, or completely torn. In everyday use, people may call any of these a “tear”; the words alone do not establish how much damage there is. The American Academy of Orthopaedic Surgeons (AAOS) describes strains across this range (AAOS: Hamstring Muscle Injuries).
A particularly serious injury occurs when a tendon pulls away from the bone. This is called an avulsion; sometimes a piece of bone is pulled away with it. A clinician can assess the injury and determine whether further evaluation or specialist care is needed.
Symptoms and what injury grades mean
Hamstring injuries often cause sudden, sharp pain in the back of the thigh, particularly during running or sprinting. Tenderness, swelling, bruising, weakness, and difficulty standing or walking may follow. A snapping sensation or audible pop can occur in a more severe injury, but no single symptom proves a particular grade. Symptoms also overlap between grades, so the descriptions below are a guide, not a way to diagnose yourself.

This three-grade framework is a broad clinical guide. For example, AAOS notes that a strain can range from a pull to a complete tear, while nidirect describes the common symptom patterns by grade (nidirect: Hamstring injury). Do not use the table as a home scoring system.
When to seek medical advice
Seek prompt assessment for severe pain, extensive bruising, marked weakness, or inability to walk or use the leg normally. These features can accompany a serious injury, including an avulsion, and should not be managed by trying to identify a grade at home.

For readers in the UK, NHS guidance advises contacting NHS 111 for severe or worsening pain, substantial or worsening swelling or bruising, pain when standing or walking, marked stiffness, or symptoms that do not improve with self-care. Elsewhere, use the local urgent-care or medical-advice service (NHS: Hamstring injury).
How long does recovery take?
Recovery depends on the extent and location of the injury, how function returns, and whether a tendon avulsion or surgery is involved. Mild injuries may settle over days; more severe injuries may take weeks or months. Symptoms at the start do not provide a dependable healing timetable, and there is no single recovery duration that applies to every hamstring injury.

Surgical repair has a distinct rehabilitation course, not a routine strain-recovery estimate. AAOS says rehabilitation after proximal hamstring tendon repair typically takes at least six months; after distal repair, return to athletic activity is approximately three months (AAOS: Hamstring Muscle Injuries).
What to do in the first few days
For mild-to-moderate injuries, NHS guidance recommends PRICE during the first two to three days: protect the leg, rest from activities that aggravate it, ice, compress, and elevate. This is initial self-care, not a substitute for assessment when symptoms are severe or worsening.

- Protect and rest: Stop or reduce activities that trigger pain; avoid pushing through it.
- Apply cold: Wrap an ice pack or a bag of frozen vegetables in a towel and apply for up to 20 minutes every two to three hours. Do not put ice directly on the skin.
- Compress: Use a snug elastic or tubular bandage, but not one that is tight.
- Elevate: Raise the leg when practical to help manage swelling.
- Avoid heat, massage, and alcohol: NHS guidance recommends avoiding these during the first three days.
Begin gentle movement when pain allows. If recovery is slow or injuries recur, physiotherapy may help; severe injuries may need specialist care (NHS: Hamstring injury).

Do not choose a return date from the injury grade or the initial pain alone. AAOS advises returning to sport only after strength and flexibility have recovered (AAOS: Hamstring Muscle Injuries). International consensus guidance also supports tailoring rehabilitation to the athlete, sport, affected muscle, and injury severity. Progression should take account of symptoms, strength, and how the leg responds to loading; later stages commonly build toward the running and sprinting demands of the sport (International consensus statement on hamstring injury in sport).

Reinjury is a meaningful concern: authors of a 2018 rehabilitation review reported that nearly one third of hamstring strains recur within the first year after return to sport (2018 rehabilitation review, PubMed). That figure is a review finding, not a prediction for any one person, but it reinforces the importance of progressing rehabilitation rather than returning solely because pain has eased.
