A hamstring strain can be a minor pull, a partial tear, or a complete tear. The terms “strain” and “tear” therefore overlap; the useful distinctions are how much tissue is damaged, where the injury is, and how much strength and movement it affects. A tendon avulsion, in which a tendon pulls away from bone, is a more serious pattern that needs prompt medical assessment.
What is the difference between a hamstring strain and a tear?
“Strain” is a broad name for an injury to the hamstring muscles and their tendons. It does not mean the tissue is intact: a strain can involve a small amount of fiber damage, a partial tear, or a complete tear. The American Academy of Orthopaedic Surgeons (AAOS) describes this range in its hamstring injury guidance.
Severity also depends on location. Damage may be in the muscle belly, where the muscle fibers are; at the muscle-tendon junction; or in a tendon near its attachment to bone. In a tendon avulsion, the tendon pulls away from bone and may take a small piece of bone with it. That differs from a tear confined to the muscle belly and can require specialist care.
What does a hamstring strain or tear feel like?
Common symptoms include sudden pain at the back of the thigh, tenderness, swelling, bruising that may appear over hours or days, weakness, and difficulty moving the leg or bearing weight. A pop or snapping sensation can occur with a more severe injury. These signs can suggest the extent of the problem, but symptoms alone cannot reliably establish the grade or rule out an avulsion.

How clinicians describe severity
- Grade 1: very little muscle is torn; symptoms are generally milder.
- Grade 2: a partial tear, often with pain and some loss of strength.
- Grade 3: a complete tear, commonly associated with substantial strength loss and difficulty using the leg.
This grading system is a guide, not a self-diagnosis. The injury’s location and its effect on walking and leg function matter too. Cleveland Clinic outlines these grades and symptoms in its hamstring injury overview.
How is the injury assessed?
A healthcare professional will ask how the injury happened and examine the thigh for tenderness, bruising, swelling, weakness, and difficulty moving or bearing weight. The examination helps determine whether imaging is needed and which test may be useful.

- MRI: can help assess the severity of soft-tissue damage.
- Ultrasound: may also be used to examine the injury.
The appropriate test depends on the examination and what type of injury is suspected; not everyone needs every scan.
What should you do at first?
For a mild or moderate injury, the NHS recommends PRICE care during the first 2 to 3 days: protection, rest, ice, compression, and elevation. Its hamstring injury advice gives these practical limits:

- Protect and rest: stop the activity that caused the pain and avoid movements that make it worse.
- Apply ice: wrap a cold pack in a towel and use it for up to 20 minutes every 2 to 3 hours. Do not put ice directly on the skin.
- Use compression if appropriate: a bandage should feel snug, not tight, and should be removed before sleep.
- Elevate the leg: raise it when practical to help limit swelling.
During the first three days, the NHS advises avoiding heat, massage, and alcohol because they may increase swelling. Gentle movement can resume when pain no longer prevents it. Seek medical advice rather than relying on home care if symptoms are severe or worsening.
How does treatment differ by severity?
Mild or moderate muscle injury
Most hamstring strains improve without surgery. Care generally moves from easing pain and swelling to progressive rehabilitation. AAOS describes physical therapy that begins with flexibility work and adds strengthening as healing progresses. The plan should match the injury and symptoms rather than a fixed timetable.

Severe tear or tendon avulsion
A severe injury may need assessment by a specialist. Surgery is most often considered for tendon avulsion injuries; AAOS says it is not commonly performed for tears within the muscle belly. The word “tear” alone does not mean surgery is needed.

Recovery varies with severity and the injury’s location. Cleveland Clinic gives a broad range from a few days to several months, while the NHS notes that severe injuries may take weeks or months. These are general ranges, not a promise of when an individual will be ready.

Return to running, sport, or other demanding activity should be gradual and guided by a clinician or physical therapist. AAOS advises against returning before medical clearance: going back too soon can lead to reinjury and lasting problems. Follow the rehabilitation plan rather than using pain improvement alone as proof the hamstring is ready.
When should you seek medical help?
Get urgent medical advice if pain is very severe or worsening, swelling or bruising is substantial or getting worse, walking or standing hurts, movement is very stiff or difficult, or self-care is not helping. Cleveland Clinic advises emergency evaluation for severe pain or inability to move or use the leg, and recommends contacting a healthcare provider if symptoms persist for more than a few days.
