A footballer can safely return only when their treating medical team has cleared them against individual clinical and football-specific criteria and they feel mentally ready for full training or match availability. There is no fixed number of days—or single scan or test—that establishes readiness. The decision follows a graded progression from rehabilitation to running, sprinting, football-specific work and, finally, full availability.
Why there is no universal return date
Hamstring injuries differ in type, severity and recovery. A calendar estimate cannot account for how an individual player is healing or whether they can meet the demands of their role. The worldwide football Delphi consensus defined return to play as the point when a player has received criteria-based medical clearance and is mentally ready for full availability for match selection and/or full training. That consensus involved 58 experts selected through 28 FIFA Medical Centres of Excellence (van der Horst et al., 2017).
This is a structured way to make a clinical decision, not a validated guarantee that injury will not recur. A study that invited all 92 professional football clubs in England received responses from 20 clubs (21.7%) and found agreement on assessment domains, while noting that validated return-to-play criteria were lacking (Zambaldi, Beasley and Rushton, 2017).
What clinicians assess before clearance
Readiness is judged across several domains rather than by one result. The football consensus includes medical-team clearance, no pain on palpation or during strength, flexibility and functional testing, comparable hamstring flexibility, field performance and psychological readiness.

- Pain and examination: The clinician checks for pain when examining the injured area and during relevant testing.
- Strength and flexibility: Assessment considers hamstring function and flexibility. Between-limb strength comparisons may inform the picture, but a symmetry figure alone is not a universal pass/fail threshold.
- Football-specific function: Field work can include repeated sprint ability, deceleration drills and other explosive tasks. Position-specific, GPS-targeted, match-specific rehabilitation may help reflect the player’s actual demands.
- Confidence and mental readiness: The player should be assessed for readiness to perform fully, not just for physical signs of recovery.
A 2026 systematic review of 135 studies on return-to-play criteria after lower-limb muscle injuries in male soccer players found that strength and pain were commonly cited after hamstring injuries, including between-limb knee-flexor/extensor strength comparisons and absence of pain during soccer-specific drills. Its database search ran through 10 March 2024; the review describes criteria used in studies, not a universally validated threshold (van Dyk et al., 2026).
How the return progression works
Rehabilitation should progress toward the actual demands of football rather than jump from basic exercise straight to match play. The London International Consensus on hamstring rehabilitation addressed exercise selection and dosage, criteria for progression, running and sprinting, and return-to-sport criteria (London International Consensus, 2022).

- Rehabilitation: Build capacity through the exercises and loading selected by the treating clinician. Progression considers symptoms, tissue healing and injury type.
- Running, then sprinting: Increase running demands and assess the player’s ability to sprint. Sprint ability was prominent in the professional-football Delphi study, reflecting its importance to the game.
- Explosive and football-specific field work: Add relevant acceleration, deceleration, repeated-sprint and position-specific tasks as appropriate. Assess performance and symptoms during and after these demands.
- Full training and availability: The medical team considers whether the player is ready for full training and match-selection availability, alongside the player’s confidence and input from relevant performance and coaching staff.
The 2022 consensus reported agreement ranges of 78.8%–96.3% for exercise selection and dosage, 73%–92.7% for progression criteria, 83%–100% for running and sprinting, and 78.3%–98.3% for return-to-sport criteria. These figures describe expert agreement, not recovery probabilities or guarantees of safety.
Why a scan or one successful test is not enough
An MRI result should not be used to decide readiness, according to the worldwide football consensus. Likewise, the absence of pain by itself, a strength comparison, one completed drill or even a match appearance cannot independently establish that a player is ready. The decision combines examination, functional performance and psychological readiness in the context of the player’s recovery.

Where reliable pre-injury performance data exist, clinicians may compare current performance with that baseline; comparison with the opposite limb may also be relevant. Neither comparison is an automatic clearance rule.

The decision should be collaborative. The treating sports physician or physiotherapist assesses the injury and progression with the player and relevant sports medicine, physiotherapy, fitness and coaching staff. The player’s symptoms and confidence matter, but clearance is a medical decision informed by the demands of full training and the player’s position—not a decision based solely on a coach’s timetable or the player’s wish to play.
