Heartworm vs. Silent Hill: How Their Psychological Horror Differs

Heartworm takes inspiration from late-1990s Silent Hill, but Sam’s grief, memory-driven spaces, fixed cameras, and camera-based mechanics give it a distinct identity. Compare it with SILENT HILL 2 and the series’ changing approach.

Heartworm shares a clear lineage with Silent Hill, but it is not simply a new version of the same nightmare. Both turn inner distress into horror through place and atmosphere; Heartworm centers Sam’s grief and memories, while SILENT HILL 2 frames its story around James Sunderland’s guilt, grief, and existential unease. The distinction matters because Silent Hill is a franchise whose entries use different stories and horror techniques.

What is the central difference between Heartworm and Silent Hill?

Heartworm follows Sam, who is consumed by grief after losing people close to her. She pursues an internet rumor about a house connected to the afterlife, driven by the hope of contacting the dead. Its official description focuses on the memories people keep and those they wish they could forget. Heartworm’s official site calls it a psychologically focused love letter to late-1990s Silent Hill and Resident Evil—an acknowledged influence, not a claim that the games share a plot or use horror in identical ways.

For a specific comparison, SILENT HILL 2 is the closest anchor: Konami describes James Sunderland’s experience in terms of guilt, grief, and existential horror, with the town serving as a symbolic backdrop wrapped in fog. Konami’s SILENT HILL 2 page supports that reading. The overlap is the use of psychological distress as part of the horror; the emotional starting points and their expression differ.

How do grief, memory, and setting shape the horror?

Heartworm: memory made into a place

Heartworm combines a surreal suburban setting with environments that shift away from ordinary geography. Creator Vincent Adinoffi has said that media helps chronicle memory and that liminal spaces connect otherwise unrelated areas. He described these environments as possible glimpses into Sam’s psyche and as a form of environmental storytelling. The spaces therefore work both as places to explore and as an indirect way of expressing her inner life. Adinoffi’s interview with Game Developer explains this stated design intent.

SILENT HILL 2: a town as symbolic backdrop

In SILENT HILL 2, Konami likewise describes the town as symbolic and links the horror to James’s guilt and grief. That creates a useful point of comparison with Heartworm: both associate place with a character’s inner experience. It does not establish that the locations carry the same symbolism or that the stories make the same argument.

Nor should the SILENT HILL 2 approach be treated as the rule for the whole series. Silent Hill’s stories and techniques vary by entry; the shared thread is psychological horror, not one fixed set of themes or settings.

How do camera, controls, and mechanics affect the experience?

Heartworm: fixed cameras and a camera used as a tool

Heartworm draws on fixed-camera survival horror and retro, low-resolution 3D imagery. It offers optional classic tank controls alongside modern controls. Exploration and puzzles are central stated mechanics, while Sam’s camera functions as both a light source and a weapon. That combination ties the game’s visual identity to its action: the camera is not only how the player sees the world, but also part of how Sam navigates danger. Its original soundtrack contributes to the atmosphere. The Steam listing describes its mechanics and estimates four to six hours of play with multiple endings; that is the storefront’s estimate, not a measured playtime for every player.

Silent Hill changes perspective by game and version

Perspective is one visible way the series has changed. The SILENT HILL 2 remake uses an over-the-shoulder view, while SILENT HILL: Townfall is first-person. Neither should be projected onto every Silent Hill release, and the fixed-camera design of Heartworm is not a universal template for the series.

Is Heartworm copying Silent Hill?

Heartworm openly identifies late-1990s Silent Hill as an influence, alongside Resident Evil. Its fixed cameras, retro presentation, survival-horror structure, and psychological focus make that connection legible. But influence is not equivalence: Sam’s grief, the internet rumor, the camera-based mechanics, and Adinoffi’s stated interest in memory and liminal spaces give Heartworm its own design identity.

The broader series history also resists a simple one-to-one comparison. In an archived interview, series producer Akira Yamaoka described Silent Hill as originally conceived around “inner fear” and psychological terror, while noting that a later entry could retain that core and aim for a different atmosphere. In the same interview, he characterized Silent Hill 4 as emphasizing the specific fears of being chased and trapped. What does a newer Silent Hill game show about the franchise?

SILENT HILL: Townfall, released September 24, 2026, is a current example of the series taking a different formal route: Konami describes it as first-person psychological horror built around exploration, evasion, combat, and narrative-linked puzzles. Heartworm launched on Steam on July 31, 2025. Recent platform reporting lists releases on PS5, PS4, Xbox Series X|S, Xbox One, Nintendo Switch 2, and Nintendo Switch; availability can differ by region and should be checked in the relevant store. Which game should you choose?

  • Choose Heartworm if you want a compact, retro-styled survival-horror game built around grief, memory, fixed-camera framing, exploration, and camera-based combat.
  • Choose SILENT HILL 2 if you want the series’ focused story of guilt, grief, and existential horror, presented in its remake’s over-the-shoulder format.
  • Look at another Silent Hill entry if you are interested in how the franchise changes its perspective and horror techniques from game to game; Townfall is a first-person example, not a stand-in for the full series.
