The Helix Fossil revives into Omanyte, which evolves into Omastar. The Dome Fossil revives into Kabuto, which evolves into Kabutops. Which one you can obtain—and how—depends on the Pokémon game and version you are playing.
What Pokémon do the fossils revive?

That Pokémon and its evolution line are the core difference. The fossils do not produce alternate forms of the same Pokémon: choosing Helix leads to the Omanyte line, while choosing Dome leads to the Kabuto line.
Does the choice affect availability?
Yes, but the rules are game-specific. In Pokémon Brilliant Diamond and Pokémon Shining Pearl, the official Trainers Guide says Helix and Dome Fossils can be dug up in the Grand Underground after you receive the National Pokédex. Before that point, the guide says Brilliant Diamond makes the Skull Fossil available, while Shining Pearl makes the Armor Fossil available.
That example does not describe every game. Fossils may be obtained or revived through different methods in other titles. Serebii’s Helix Fossil and Dome Fossil references list revival locations across several games; for exact steps, check instructions for the specific title and version you are playing.

Which fossil should you choose?
If both are available and you can take only one, choose based on which Pokémon line you want: Helix for Omanyte and Omastar, or Dome for Kabuto and Kabutops. If the game limits your initial choice or makes one fossil harder to obtain, that is a rule of that particular title, not a universal difference between the fossils.
Are the fossils different in the Pokémon Trading Card Game?
Yes. The Trading Card Game has its own fossil-card rules, separate from video-game revival. The official Pokémon TCG rulebook describes Helix Fossil Omanyte as an Item card and explains that Omanyte is a Restored Pokémon that enters play through that card’s effect; it also uses Kabuto as an example of a Restored Pokémon. Those rules do not explain where to find or how to revive fossils in a video game.

What do the fossil designs refer to?
The names and designs evoke real prehistoric life, but the Pokémon should not be treated as exact real-world species. A 2017 article by the North Dakota Geological Survey notes that the Dome Fossil can initially resemble a trilobite and says the inspiration behind the Helix Fossil is easier to recognize. That supports a comparison of visual inspirations, not a claim that either fictional Pokémon is literally a particular fossil species.
