There are six Vaults of Forbidden Knowledge in Hell Is Us, one in each of six regions. This spoiler-forward guide gives the route to every vault, its four-symbol dial sequence in order, and the reported contents. Codes use symbols—not numbers.
How vault clues and codes work
Submitting Research Items to Tania in the APC is the intended way to receive clues to vault locations and combinations. The solutions are fixed, however, so you can use the sequences below as soon as you can reach the relevant area; the vaults can also be opened after the story. These mechanics are reported by PowerPyx and Vandal.

Each code is a four-symbol sequence. Read it from left to right and select each dial symbol in that order. Where guides identify a symbol by its drawing, the corresponding name is included here: figure eight is Admiration, wide-open eye is Amazement, eye with a diagonal is Vigilance, and U-shape is Loathing. The mapping is cross-checked against PCGamesN.

From the APC, head uphill toward the OMSIF/Field Camp near the top. Head from the APC toward the waterfall and stone ruins. Reach the waterfall’s upper ledge, then drop to its middle level and pass through or behind the waterfall into the tunnel. What to know about the reward lists
The reported contents mix combat gear and upgrades with lore and collectibles. Exact item-by-item lists are not fully consistent between the walkthroughs: the Mere prism is called Zeta by Vandal and transcribed as “Thetha” by PowerPyx, while only Vandal includes a Sword of Rage in the Prairie vault. Check the in-game item label if you need to confirm either item by name or verify a complete inventory.
