Hellish Descent is described in a Diablo IV guide as a dungeon in Scosglen’s Wailing Hills, with Penitent difficulty listed as its requirement. Its route reportedly runs from Door Wardens and sacrificial altars to a Hell Breach and a final encounter—but the available guide contains a conflicting boss name, and the access, rewards, and seasonal unlocks are not confirmed by official Blizzard documentation. Use the in-game objectives as the authority if your version differs.
How do I unlock and enter the Hellish Descent dungeon?
A Blizzard forum post asks why Hellish Descent does not appear and how to unlock it, but the post does not establish an unlock condition. A separate guide lists Penitent difficulty as a requirement and places the dungeon in Wailing Hills, Scosglen; those are reported details, not confirmed current requirements. See the forum post about Hellish Descent and the guide listing the dungeon.
If it is missing from your map, check the selected difficulty and the current in-game quest or seasonal objectives first. The available evidence does not say whether another prerequisite, campaign state, or seasonal progression step is needed. Do not treat Penitent difficulty alone as a proven unlock method.
What are the reported Hellish Descent objectives?
The secondary guide gives this sequence. Dungeon objectives can change between patches or seasons, so follow the live objective tracker if it differs from this route.
#1 Best Overall
- Defeat three Door Wardens.
- Defeat the Hellfire Adherent.
- Complete three Sacrificial Altars.
- Summon a Hell Breach.
- Destroy three Demonic Corruptions.
- Proceed to the boss encounters described below.
How do the Sacrificial Altars and Hell Breach work?
The guide says to lure enemies into the red circles around the Sacrificial Altars and kill them there to advance altar progress. It does not give a confirmed count of enemies or explain whether every altar uses identical rules. Watch the objective text and the altar indicator rather than assuming a particular number of kills.
After the altar stage, the same guide lists summoning a Hell Breach and destroying three Demonic Corruptions. It does not provide a verified explanation of the breach interaction, so use the prompts and markers shown in your instance.

Which bosses appear, and what if Tordak does not spawn?
The guide lists Xol’noct and Tordak after the corruption objectives, but later refers to the final boss as “Elias.” Because those names conflict within the same guide, the exact current boss order is not established. Check the encounter names and objectives displayed in your game rather than relying on the inconsistent list.
A separate Blizzard forum post reports “Tordak won’t spawn, only the Butcher,” describing a Burning Butcher Infernal Hunter appearing in the final room. That is one player’s report, not evidence that this is a widespread or currently recognized bug. If your encounter matches it, record the objective state and report it through Blizzard’s support or bug-report channels; the available sources do not establish a confirmed workaround.

What does completing Hellish Descent unlock?
The guide associates completion with Season Rank III, The Pit, and the Tower, and reports two skill points and one Smoldering Ash as rewards. These are season-sensitive claims and are not corroborated by official documentation in the available sources. Check the current season’s in-game reward and progression panels before entering specifically for one of these unlocks.
The guide also recommends increasing fire resistance because of reported meteor damage. That mechanic is not confirmed by an official source here, so consider your build’s defenses generally and respond to the attacks actually used in your run.

Could “Hellish Descent” mean a different game?
The same title is used by Scratch’s Sc0re: Hellish Descent, a downloadable card-game expansion listed on itch.io. Its listing says it expands the 2019 base game, requires that base game, and includes print-and-play files, including a printer-friendly version. If you meant the card-game expansion rather than the Diablo IV dungeon, see its itch.io listing; the dungeon walkthrough above does not apply.
