Under the NBA’s 2025–26 concussion policy, a player with a suspected concussion is removed and evaluated by team medical staff. If diagnosed, the player must complete a monitored, step-by-step return process and cannot resume unrestricted full participation for at least 48 hours after the injury. There is no fixed number of days to finish the process; return depends on symptoms, medical clearance and league confirmation.
What happens when an NBA player may have a concussion?
A team physician or athletic trainer must remove a player who is suspected of having a concussion or shows signs or symptoms. The initial assessment takes place in a quiet setting without distractions. Before the player can return to game participation after that evaluation, the evaluating physician or athletic trainer reviews video to assess how the injury happened.
A negative initial evaluation does not end the process. Team medical staff continue monitoring and must conduct at least one further concussion evaluation before the next team game or practice, or about 24 hours after the initial evaluation, whichever comes first. If new signs or symptoms appear, the player must be removed immediately and evaluated again.
In either a diagnosed or non-diagnosed case, a physician experienced in sports-related concussion must review whether a concussion is present within 24 hours. A physician ultimately confirms the diagnosis and takes part in the management plan. Team medical staff must also notify the NBA Concussion Program Director of the evaluation and its result within 24 hours. These requirements are set out in the NBA Concussion Policy Summary for the 2025–26 season.

How does recovery begin after a diagnosed concussion?
The NBA policy calls for limiting physical and cognitive exertion under team medical supervision. After a brief rest period of 24–48 hours, activity may gradually increase under the team physician’s direction, provided it does not bring on symptoms or make them worse. The policy also addresses sleep, nutrition and hydration, and limiting electronic games and devices, time in large groups, and other demands until medical staff provide clearance.
What are the return-to-participation stages?
The player is monitored for evolving symptoms for 24 hours and cannot begin the exertion process until at least 24 hours after the injury. The team physician decides when it is appropriate to start. Medical staff directly monitor each stage and conduct a focused neurological examination at every step.

The player must be symptom-free to advance. If symptoms occur, activity stops until the player is symptom-free; the player then resumes at the last stage passed without symptoms. The policy does not set a fixed timeframe for completing the stages.
Why are 24 and 48 hours not a return date?
The 24-hour rule is the earliest point at which the exertion process may begin. Separately, the player cannot resume unrestricted full participation until at least 48 hours after the injury—and that minimum does not replace the required stages, symptom checks, physician clearance or NBA confirmation. The NBA’s policy says there is no set completion timeframe because injuries and recovery vary.

Who clears a player to return without restrictions?
Before unrestricted full participation, the player must be symptom-free at rest, be assessed by a physician experienced in sports-related concussion, complete the exertion process and receive physician clearance. The NBA Concussion Program Director must then confirm the clearance is consistent with policy. A coach does not make the medical clearance decision.
If the team physician believes the player may return but the Program Director disagrees, the physician can submit a written opinion and rationale. An independent neurologist designated by the league reviews it. The policy calls for that review no later than 24 hours after the team submits the opinion, or sooner when game conditions require and reasonable time for review is available.

What the policy does—and does not—say about recovery time
The NBA’s 2025–26 summary does not publish an average recovery duration or a standard number of days out. Each case depends on the player’s symptoms, progress through the stages, medical assessment and completion of league requirements. A general athlete statistic should not be mistaken for an NBA-player forecast: the Jr. NBA page, dated October 8, 2015, says studies report that symptoms improve in 80–90% of athletes by seven days, but it does not identify those studies in the page text and does not present an NBA-player return estimate (Jr. NBA concussion education).
The current process is the NBA’s 2025–26 policy. The league’s 2011 announcement describes the program’s early framework, including annual education, baseline assessments, a multistage protocol and discussion of return decisions with the program director; it is historical background rather than the current procedural authority.
