Look at the Dungeon entrance: if the Cultists have spawned, you’ll see four hooded figures beside a Mysterious Tablet—two Cultist Archers and two Lunatic Devotees. They are passive until attacked, so you can identify them without starting a fight.
What the Cultists’ spawn looks like
The group appears at the Dungeon entrance around the Mysterious Tablet. The two Archers and two Devotees stand there as distinct hooded figures; their presence is the clearest sign the spawn has occurred.
Before any Cultist is attacked, the group is passive, deals no contact damage, and can be walked through. Attacking one makes the Archers hostile and causes them to shoot arrows, but you do not need to provoke them to confirm they are there. The Guide describes the clue as “There are some strange people in hoods loitering outside of the Dungeon. Maybe you should check it out?” (Official Terraria Wiki: Cultists).
Why the Cultists might not be there
The Cultists are a Hardmode spawn. The Official Terraria Wiki lists these conditions: Skeletron and Golem must have been defeated; no boss can be alive and no event can be in progress; a Mysterious Tablet must not already exist; and the player must be outside a rectangular area around the Dungeon point, described as 144 tiles wide by 81 tiles high (Official Terraria Wiki: Cultists).

Check progression and temporary blockers
- Confirm the world is in Hardmode and that both Skeletron and Golem have been defeated.
- Make sure no boss is alive and no event is happening.
- Check whether a Mysterious Tablet is already present.
- Move away from the Dungeon point, beyond the area the wiki describes as 144 tiles wide by 81 tiles high.
Check the Dungeon entrance floor
Removing the floor at the Dungeon entrance where the Old Man appears prevents the Cultists from spawning, according to the Official Terraria Wiki (Official Terraria Wiki: Lunatic Cultist). If the progression and temporary conditions are clear, inspect that original entrance area and restore its floor if it has been removed.

Defeating all four Cultists summons the Lunatic Cultist at their location and destroys the Mysterious Tablet. The boss briefly appears at the tablet during the summoning animation, then begins the fight (Official Terraria Wiki: Cultists; Official Terraria Wiki: Lunatic Cultist).
If the Cultists are not summoned, they reset and return after 12 minutes—equivalent to 12 in-game hours—according to the Official Terraria Wiki. If the Lunatic Cultist has already been summoned but the fight is lost or the boss despawns, wait for the group to return before attempting another summon.
