You cannot reliably tell whether someone is a Gen 3 synth by looking at them in Fallout 4. Covenant’s SAFE test is an unreliable in-world experiment, while the Suspected Synth quest offers only limited clues. The clearest confirmation—a synth component—can be found only after the accused person is dead.
Why appearance cannot identify a Gen 3 synth
Gen 3 synths are designed to be physically indistinguishable from humans in ordinary examination. Covenant researcher Dr. Roslyn Chambers puts it this way: “A living synth is indistinguishable from a human by any medical test yet devised.” That is fictional dialogue describing the game’s premise, not a real-world scientific claim. That means facial features and other ordinary observations do not provide a dependable visual test. The game does not offer a general, reliable way to identify synth settlers through appearance or everyday behavior.

Why Covenant’s SAFE test is not proof
Covenant residents use the SAFE test to screen visitors. In the quest, it is part of an ongoing experiment into whether the assessment can detect synths—not a validated verdict. The player character cannot fail the entry test, so the result does not establish whether the player is a synth. Treat SAFE as part of Covenant’s story and its fear-driven choices, not as a dependable tool for identifying people. Human Error quest reference
Clues in the Suspected Synth quest
When the radiant Suspected Synth quest asks you to investigate an accused settler, the game provides a small number of mechanics-based clues. A hard dialogue check may be available. If you have the Awareness perk, increased Energy Resistance is another clue. Neither is described as conclusive. The quest reference characterizes the accused person as having a 50/50 chance of actually being a synth; that describes this quest scenario, not a general probability for settlers or every game state. A synth component on the corpse is the clearest listed confirmation. But obtaining it requires killing the accused person, so it is not a nonlethal diagnostic. The game does not supply ordinary, harmless proof that resolves every suspicion.

There is no universally best choice established by these clues. Covenant’s screening can harm innocent people, and the strongest confirmation in the Suspected Synth quest is fatal. Decide according to your character’s roleplay and quest goals rather than treating suspicion as certainty. The available quest clues do not support using sleeping habits, food preferences, or other community theories as reliable tests.
