Dark Messiah of Might & Magic has four ending variations. They combine two final choices about the Skull of Shadows with two possible companions in the closing scene: Leanna or Xana.
What determines the four endings?
The count is best understood as two separate factors, rather than four unrelated final decisions. The final Skull of Shadows choice determines Kha-Beleth’s fate; the companion variation determines who appears with Sareth in the closing scene. What is the final choice?
After defeating Arantir, Sareth decides what to do with the Skull of Shadows. Using it to imprison his father, Kha-Beleth, is described by GameSpot’s walkthrough as the good ending; destroying it and freeing Kha-Beleth is described as the bad ending. Those labels refer to the main outcome, not to the companion variation.

How do earlier choices affect the companion?
Earlier decisions involving Leanna—including whether to rescue her and whether Sareth undergoes the cleansing ritual—affect whether she can accompany him later. If she is not the companion in the final scene, Xana is the alternative. The walkthrough says the earlier Leanna decision does not prevent Sareth from reaching either of the two main Skull outcomes.

Why do some answers describe fewer endings?
Counting only the two possible fates of Kha-Beleth gives two main outcomes. Counting the companion-dependent closing scenes as well gives four ending variations. The developer’s GameSpot developer Q&A confirms the broader idea that actions during the game determine several endings, while the specific two-by-two breakdown comes from the walkthrough and community-maintained game reference.
