NFL injury designations tell you how likely a player is to participate—not whether he is a good fantasy start or how much he will play. Treat Out as a replacement decision, Doubtful as a strong warning to prepare one, and Questionable as unresolved uncertainty. Then check the latest official update, your league’s lineup-lock rules and the player’s inactive status before kickoff.
What do Questionable, Doubtful and Out mean?
The NFL’s revised injury-report policy defines the labels by expected game availability. They are not fantasy projections or published probability estimates.

The NFL removed “Probable” as a game-status category. The league said approximately 95 percent of players previously listed as Probable played; that historical figure explains the policy change and is not a current estimate for players listed Questionable or Doubtful. How practice reports differ from game status
Practice participation is reported as Did Not Participate, Limited Participation or Full Participation. The NFL defines Limited as fewer than 100 percent of normal repetitions and Full as 100 percent. These describe practice; the separate Game Status Report communicates anticipated availability for the game. Midweek practice is useful context, but it does not replace the game-status report.

For the 2026 NFL calendar, reporting deadlines depend on the scheduled game day. For a Sunday game, the Game Status Report is due Friday; other game days have different deadlines. Clubs must report changes after filing the initial report, so an earlier snapshot may no longer reflect the player’s status. Check the league’s Important Dates calendar for the applicable schedule and deadlines.
How to make the lineup decision
- Start with the designation. Sit an Out player if your lineup is still editable. Treat Doubtful as a strong reason to plan on a replacement. A Questionable tag means uncertainty remains, not that the player must be benched.
- Check the latest update. Use current team or league information rather than relying on a stale practice report. The game-status report can change after its initial filing.
- Work backward from kickoff and lineup lock. Decide before the player’s game locks in your platform. A later kickoff can leave time to use a backup from a later window, but league and platform lock rules vary; check your own settings.
- Name a usable contingency. Identify an eligible bench player who will still be available when you must decide. Compare that player’s expected role and opportunity using current information; the injury designation by itself says nothing about workload.
- Check the inactive list near kickoff. NFL teams designate inactive players about 90 minutes before the scheduled game. That is a late checkpoint and may arrive too late to switch to some alternatives, depending on their kickoff times and your league rules.
What a designation cannot tell you
Neither Questionable nor Doubtful supplies a fixed play probability, a fantasy-points projection or a snap-count estimate. An active player may still have an uncertain role or workload. Evaluate availability separately from expected production: consider the latest report, the player’s role, any credible workload information and the quality of your alternative. No current probability estimate for Questionable or Doubtful players is established by the NFL policy source.

Compare the player with the actual alternative
The useful choice is rarely “injured player or no risk.” It is usually one player against a specific replacement, under your roster and league rules. Weigh these factors together:
- Availability: designation, how recently it was updated and whether the inactive announcement is still pending.
- Backup access: whether you have an eligible option and whether that option’s game starts later.
- Expected role: starter or reserve status, recent use and credible evidence of a workload limit. The designation alone does not establish any of these.
- League mechanics: position eligibility, lineup lock, waivers, injury-slot rules and platform features. These differ by platform and league.
- Roster context: a viable bench option can reduce exposure to a late inactive; a thin position or deep league may make keeping a higher-upside player more attractive. Neither choice guarantees a result.
What fantasy auto-swap can—and cannot—cover
NFL Fantasy’s Auto-Swap Inactives feature uses the manager’s ranked backup list and swaps only players projected to score zero points.
