There is no single league-wide formula for when an NFL team starts its backup quarterback. Coaches make a week-specific call based on the starter’s health and ability to run the offense, the backup’s preparation and performance, staff confidence, and what best serves the team’s immediate and season-long goals.
First, can the starter safely and effectively play?
When an injury is involved, the team’s medical process determines availability. But being cleared does not necessarily settle who starts: coaches also consider whether the quarterback can carry out the offense without being so limited that the team must alter its plan.
Steelers head coach Mike Tomlin described the distinction while discussing Russell Wilson: medical staff assess health and protection from further harm, while coaches judge whether the quarterback can perform productively. He cited pocket movement, play-action, reaction speed, and use of the legs, summing it up as “Health and then his ability to do all the things that we ask someone in his position to do.” Tomlin’s October 2024 comments illustrate the factors he said mattered; they are not a universal medical threshold.
Public injury updates are not a basis for diagnosing a player. A team’s report or coach’s statement can describe status, but it does not reveal every clinical consideration or establish that clearance means full functional readiness.

How do coaches compare the backup with the starter?
Staffs can weigh practice preparation, familiarity with the game plan, recent play, and how confidently they believe each quarterback can execute the offense that week. A backup may be chosen because the coaches think he gives the team a better chance to win—not simply because the starter made a mistake or because the backup has never had an opportunity.
Preparation and performance can decide the week
In 2024, Giants head coach Brian Daboll said Tommy DeVito and Drew Lock had done good work in their assigned roles and preparation. After coaches discussed the decision over several days, they selected DeVito based on their evaluation of player performance, confidence in him, and the spark he had brought the team the previous year. Confidence and conviction are part of the judgment
In 2025, Dolphins coach Mike McDaniel said Quinn Ewers gave Miami its best chance to win the next game and that he expected more conviction from the quarterback position. For that game, Ewers was the starter, Zach Wilson the backup, and Tua Tagovailoa the emergency third quarterback. Those were that week’s roles, not a current depth chart. McDaniel’s announcement and explanation show how a coach may publicly frame the choice around his view of what the team needs in the immediate game.
Does a strong backup performance automatically take the job?
No. An effective fill-in can strengthen the case for himself without automatically displacing an established starter once that player is healthy. The staff may value the starter’s prior performance, fit, and ability to run the offense at his normal level.

During Brock Purdy’s recovery in 2025, 49ers coach Kyle Shanahan praised Mac Jones’s play but said Jones’s success would not change the decision if Purdy was healthy and able to return at his expected level. Asked whether Jones’s performance might persuade him to keep the backup in, Shanahan said, “No. It’s not a hard decision,” adding, “It’s just more about Brock’s health.” That was Shanahan’s explanation of the 49ers’ situation, not a general rule that a starter always resumes the job. Washington’s 2022 switch from Carson Wentz to Taylor Heinicke shows how recent team results and offensive approach may enter the conversation. Head coach Ron Rivera said he was comfortable with Heinicke as the starter and Wentz as the primary backup after Wentz was medically cleared. Rivera also pointed to the team’s run game, possession, and keeping the defense fresh. NFL.com reported Washington went 2-4 with Wentz and 5-1-1 after Heinicke took over that season; those are historical records, not proof that the quarterback change alone caused the difference. What else affects the decision?
The choice is about more than comparing two quarterbacks’ box scores. Coaches can consider whether each player fits the current offense, the opponent and game plan, the team’s recent performance, and the season objective. They may also discuss how a quarterback’s strengths and limitations affect the rest of the lineup. The weight given to each factor varies by team and week; coaches do not publish a common scoring formula.
After a 2022 loss, Rivera said Washington needed to review the tape and discuss the whole situation before deciding who would start. He described a deliberate evaluation tied to giving the team its best chance to win and pursue a playoff place. Why not bring in the backup for the fourth quarter?
A fresh quarterback can sound appealing when the starter is struggling, but a change is not automatically an upgrade. The backup must be prepared for that game’s plan, and the offense may lose continuity when players switch roles. Coaches have to judge whether the change improves the chance of winning now, while weighing the starter’s ability to respond and the backup’s readiness. If the starter is healthy and still executing the offense, the staff may decide that keeping him in gives the team the better option.
When does a backup start because the starter is being rested?
Not every backup start follows an injury or a demotion. A team may intentionally rest a healthy starter to protect him for a more important game ahead. In the 2023 regular-season finale, San Francisco made Purdy inactive before its divisional-round game and started Sam Darnold. Purdy remained engaged with the game plan and helped from the sideline.
