A quest can advance through authored stages, update its journal or objectives, and make later dialogue or behavior depend on that state. The Creation Kit references describe how these systems can be built; they do not establish that every quest uses the same pattern.
How Skyrim records quest progress
A useful way to understand the system is as a chain: an action or choice may trigger an event or satisfy a condition; quest logic may then advance to a stage, run an associated result, update an objective or journal entry, or affect what happens later. This is a model of documented quest-authoring mechanisms, not a guaranteed sequence for every quest. The Conditions System documentation explains conditional behavior, while the Quest implementation guide describes stages, journal updates, objectives, aliases, and event-driven progression.
Stages are authored progress points
Quest stages mark progress points chosen by the quest author. A stage can have conditions and results, and its associated journal text can tell the player what has happened. Objectives provide another way to present current tasks. A stage number is not a universal percentage, nor does it reveal every decision the player has made: the quest’s content determines what its stages mean.
Completion is a quest state, not necessarily a permanent shutdown
The secondary Quest Data tab reference describes a completion setting associated with a stage and notes that a completed quest can still be run. Completion therefore should not be read as proof that every related quest process or behavior is frozen forever.

How choices can affect later dialogue
Conditions can determine whether dialogue plays or whether a quest is added. One documented example is requiring a quest stage to have been completed before a dialogue line becomes available. In practical terms, a conversation or other action can change quest state, and a later line can check that state before appearing. The specific branch and consequence depend on how that quest was authored; the documentation does not establish the exact flags or scripts used by every Skyrim quest.
The Conditions System page summarizes its role this way: “In general, it allows you to branch your quests, story-lines and dialogues, and everything that comes from that.” The documentation supports choice-sensitive quest design, rather than a claim that each conversation response is stored in one universal ledger.

Progress can be triggered by more than dialogue
Quest content can react to events such as picking up a quest item, a target dying, reading a book, choosing dialogue, or activating a hand-placed trigger or activator. Aliases let a quest organize actors, locations, or items under named roles for reference by quest data and related logic. The Quest implementation guide describes these mechanisms as parts of quest structure.
More elaborate world reactions can use the Story Manager, aliases, and scenes. The World Interactions tutorial gives a vanilla example involving the WICommentMagicFlames quest and a GameDaysPassed condition to control when an NPC comment can occur. It is an example of one implementation, not evidence that all NPC comments or player choices are recorded identically.

Does Skyrim have a complete choice log?
The journal and objectives are visible records of quest progress, but the cited references do not say that the journal lists every decision that the game can later use. They document quest-authoring mechanisms, especially stages and conditions; they do not describe the saved-game format or provide a comprehensive inventory of vanilla quest decisions. Accordingly, these sources do not establish that Skyrim keeps one central database of every conversation response or retains every choice indefinitely.

A later line or behavior can be made conditional on a quest stage or another state that earlier gameplay set. That is a documented way for previous progress to influence a later interaction. Which event is checked—and whether the player sees a matching journal entry—depends on the particular quest content. Without examining that quest’s data, it is not possible to identify its exact implementation from the general authoring references alone.
