Highlight a right-hand weapon slot and confirm to open its item list.

Select the sword and confirm. It is now assigned to that slot.
Switch to the sword during play
Close the menu, then use the right-hand weapon-switch control to cycle to the slot containing your sword. In the original PC Prepare to Die Edition, the default key is V for the right-hand weapon; C switches the left-hand weapon. These are edition-specific defaults, not universal controls. Equipment set in the Equipment menu can be switched during play, and the right- and left-hand weapons have separate actions. Two-handing is a grip toggle for the right-hand weapon, not another equipment slot. Nintendo Switch manual

Controls depend on platform and edition
The slot-and-item procedure is documented for Windows Remastered and PlayStation 4, while the Switch manual describes switching equipment during play. The Prepare to Die Edition PC manual provides its own keyboard defaults. Controls may differ or be remapped, so consult the in-game System or Key Settings menu for your setup.
