What DRIVER_VERIFIER_IOMANAGER_VIOLATION (0xC9) means
This stop code is not a general name for every driver crash. Microsoft defines DRIVER_VERIFIER_IOMANAGER_VIOLATION as bug check 0x000000C9, the bug-check family used when Driver Verifier detects an I/O Verification violation. Microsoft states that this bug check can occur only when Driver Verifier has been instructed to monitor one or more drivers.

Driver Verifier checks how a monitored driver interacts with Windows I/O Manager routines such as IoCallDriver, IoCompleteRequest, and IoFreeIrp. When the checks find a contract violation, Windows deliberately stops with a blue screen so the state can be captured for debugging. Parameter 1 identifies the particular violation type. The stop screen or dump can also contain an I/O error code, the driver name, and the address where the violation was detected.
That distinction matters: 0xC9 tells you that verification caught a monitored driver, not that every computer with the code needs the same replacement driver. The correct next step depends on the parameter values, the driver named in the dump, and what changed before the crash.

How to fix it when Windows still starts
If you can reach the desktop, disable verification before attempting updates, removals, or repeated restarts. Use an administrator account and follow these steps in order.
- Open Command Prompt with administrator privileges. Search for “Command Prompt,” choose the administrator option, and approve the User Account Control prompt.
- Check what Driver Verifier is currently configured to monitor by entering:
verifier /querysettings - Clear all Driver Verifier settings:
verifier /reset - Restart Windows. The reset takes effect on the next boot, so do not treat the command completing as proof that verification is already inactive.
- After the restart, keep the dump file and record the stop-code parameters before deleting logs or uninstalling software. That evidence is the best guide to the driver-specific remedy.
verifier /reset clears all Driver Verifier settings; it does not repair a defective driver. It stops Verifier from forcing another verification bug check while you investigate the underlying driver or software.
How to get out of a Driver Verifier boot loop
A loop that begins after Verifier was enabled is a recovery problem first. Do not keep selecting “restart” in normal mode if the same blue screen returns. Enter either Safe Mode or Windows Recovery Environment (WinRE), open an administrator Command Prompt there, and reset verification.
Use Safe Mode
- Open the recovery or advanced-startup menu for your Windows release. If normal startup is impossible, use the recovery options presented after Windows fails to start; the exact keys and menu names differ between releases and devices.
- Choose the startup-settings option, select Safe Mode, and sign in when Windows loads. Safe Mode starts with a limited set of drivers, which can avoid the driver that is being checked.
- Open an administrator Command Prompt and run:
verifier /querysettings verifier /reset - Restart normally. If the system reaches the desktop without the 0xC9 screen, leave Verifier disabled while you examine the dump.
Use Windows Recovery Environment
- Enter WinRE using the recovery method available on your installation media, recovery drive, or failed-startup screen. The exact route varies by Windows edition and device firmware.
- Select the troubleshooting or advanced-options area, then open Command Prompt. If WinRE asks you to select an installation or user account, choose the Windows installation that contains the problem and provide its administrator credentials.
- Run the reset command:
verifier /reset - Close Command Prompt and choose the option to continue or restart Windows.
The recovery command is the same in Safe Mode and WinRE. What changes is only where the command is run. If verifier /querysettings cannot display settings in the recovery environment, still run verifier /reset, restart, and check the settings again after Windows starts.

How to find which driver caused the I/O manager violation
The stop-code name alone does not identify a universal culprit. Start with the dump created by the crash. A kernel dump or minidump can contain the 0xC9 code, parameter 1, the I/O error code, the address involved, and the driver name.
Read the dump in WinDbg
- Open the relevant kernel dump or minidump in WinDbg.
- Run the verbose analysis command:
!analyze -v - Review the bug-check parameters, especially parameter 1, and note any module or driver name shown with the I/O verification message.
- Ask Verifier for captured statistics:
!verifier - If the analysis provides an IRP address, inspect that request packet with:
!iovirp <address>Replace
<address>with the address printed in your dump; do not type the brackets literally.
!analyze -v is the broad starting point, !verifier shows the verification data that was captured, and !iovirp is useful only when an IRP address is available. If the dump does not name a driver, preserve the parameters and the full analysis output rather than guessing from the hardware that happened to be in use.
Interpret the driver name carefully
A named third-party driver or a driver changed immediately before the crash is the first investigation target. The name is evidence, not permission to delete a random system file. Match it to the device or program that installed it, then choose an update, rollback, or removal path that can be reversed. If the dump names a Microsoft component while a recently installed third-party filter, security product, storage utility, or device package is also present, examine the recent third-party change before replacing Windows files.

Choose the repair from the evidence
Install a current signed driver
When the dump points to a third-party device driver and the vendor provides a newer signed release, obtain that release from the device or software maker. Confirm that it matches your Windows edition and the actual device model. Follow the vendor’s installation instructions, restart, and observe whether the system remains stable with Driver Verifier still off.

Do not re-enable verification for every driver merely because an update is available. The point of the dump is to narrow the test to the suspected module. Keep the old dump and the date of the change so a later crash can be compared with the original evidence.
Roll back a driver after a recent change
If the 0xC9 crash began immediately after a driver update and Windows offers a rollback for that device, use the rollback option rather than installing an unrelated package. Restart after the rollback and check whether the same driver name and parameters return. A rollback is a diagnostic step; it does not prove that the earlier version is permanently safe.
Remove software that installed an unnecessary driver
If the offending driver belongs to software you no longer need, uninstall that software using Windows’ installed-app controls or the program’s documented uninstaller. Restart, then inspect a new dump if the crash returns. Avoid manually deleting a .sys file from a system directory: that can leave services, device entries, or dependent components in an inconsistent state.
Handle a driver for essential hardware
For storage, network, display, audio, Bluetooth, USB, or other hardware that Windows needs to operate, plan the change before removing the current package. Keep the installer or recovery route available, and make the change from Safe Mode or normal Windows according to the device maker’s instructions. If the device is required to boot, do not disable or remove its driver solely because its name appears in a stack trace without checking the complete dump context.

If you enabled Driver Verifier deliberately
Driver Verifier is a testing utility, not a permanent performance setting. Microsoft warns that it can crash a computer. Its normal response to a detected violation is a bug check so a debugger can capture maximum information.
Scope the test
Test one suspected driver or a deliberately limited set instead of selecting every driver. A narrow scope makes the dump easier to interpret and reduces the chance of turning an otherwise usable installation into a boot loop.
To configure standard verification for one driver, Microsoft documents this form:

Check and end the diagnostic session
Use these documented commands when you need to inspect the configuration or finish testing:
verifier /querychecks the current verification state.verifier /querysettingsshows the configured settings and monitored drivers.verifier /resetclears all Driver Verifier settings.
After the diagnostic session, run verifier /reset and restart. Leaving a broad verification profile active after the test can cause additional intentional bug checks and make unrelated stability work harder to interpret.
What to do after resetting Verifier
- Preserve evidence: save the minidump or kernel dump, the stop-code screen, parameter values, and the WinDbg output.
- Write down the timeline: note driver, hardware, Windows, and software changes made immediately before the first crash.
- Change one thing at a time: update, roll back, or remove the driver identified by the evidence, then restart and observe.
- Keep verification off during ordinary use: only re-enable it for a scoped diagnostic test.
- Recheck after a new crash: a new dump may identify a different driver or show that the original issue was not the continuing cause.

When the crash continues with Driver Verifier disabled
A 0xC9 crash is specifically tied to a monitored-driver verification failure. If the computer continues to blue-screen after verifier /reset and a restart, first confirm that the settings are actually cleared with verifier /querysettings. Then analyze the new dump rather than treating every later crash as the original 0xC9 event.
