First identify what has the question marks and whether the affected item is custom or included with the game. Then test with mods disabled—without saving—and use the result to choose the next step.
First, identify what has question marks
Note whether the marks appear on a Sim’s skin or clothes, on objects in Build/Buy or the world, or only in thumbnails. Also note whether the problem began after a game update and whether you use mods or CC. The appearance alone does not identify a specific error or cause.
If only downloaded items are affected, investigate CC and mods first. If EA-provided items are affected too, or the issue remains when custom content is removed, a game-side problem is more plausible. EA recommends treating mod problems as troubleshooting rather than assuming a single cause.
Check the game and mod versions
Updates can make previously working mods incompatible. EA advises checking the creator’s current compatibility information and replacing an outdated mod according to the creator’s instructions. The official EA Forums index linked a current broken/updated Mods and CC list for patch 1.128 on September 22, 2026; check the EA Forums Mods and CC list for changing status.

Also check whether the game has an available update. A patch can both disrupt custom content and fix a game-side loading bug, so an update’s timing is a clue, not proof of the cause.
Test the game with mods disabled
- In the game’s settings, disable custom content and mods, then restart the game if prompted.
- Load a save and enter Live Mode. Custom content will not appear during this test; look instead for whether the question-mark symptom persists on unaffected base-game content.
- Exit through the game menu without saving. EA warns that saving while CC is disabled can remove that content from the saved game.
EA’s mod troubleshooting guidance explains that mods can cause problems after updates and recommends testing with them disabled. If the symptom disappears in the clean test, focus on mods or CC. If it remains, continue with game-side checks.

If disabling mods fixes it, isolate the file
Start by removing gameplay mods, then test; narrow down CC separately if needed. Add content back in tracked batches rather than restoring everything at once. When the problem returns, divide the failing batch and test each half. Repeat until you identify the file or combination that triggers the issue. This is commonly called the 50/50 method and follows EA’s recommended approach.
- Keep a record of which files are in each test batch.
- Check the identified creator’s page for an updated version or compatibility notice.
- Leave incompatible files out until the creator updates them or confirms a safe alternative.
If it persists in a clean test, check for a game-side fix
Review the official patch notes for your platform and version, and follow the game-repair instructions for the platform you use. For example, EA’s March 18, 2026 notes for PC version 1.122.212.1030 and Mac version 1.122.212.1230 reported a fix for pack-specific CC that failed to load and prevented other CC from loading. That example establishes that a game update can cause a specific CC loading problem; it does not show that every question-mark symptom has the same cause. See the March 18, 2026 update notes.

EA’s May 5, 2026 Laundry List reported six fixes addressing different causes of black-photo texture loss, with additional testing on paintings and photos. EA said all known black-photo bugs had been resolved at that time. This applies to those photo bugs, not every missing texture or question mark. See the May 5, 2026 Laundry List.
If the issue continues, report it with your platform, game version, a screenshot, and whether it still occurs after removing mods and CC. Those details help distinguish a content conflict from a problem that persists in a clean game.
