Windows 11 error 0x8007045D (also displayed as 0x0000045D) means ERROR_IO_DEVICE: Windows could not complete an input/output request. When it appears during System Restore, the failure can involve the system drive, another storage device, a damaged file system, a cable, USB or SATA port, enclosure, controller, or recovery media. The code alone cannot identify which part is at fault.

Protect your files first, then remove unnecessary devices, identify the affected volume, run a read-only CHKDSK scan, repair the file system only after confirming a backup, and retry System Restore from Windows Recovery Environment (WinRE). If the restore still fails, use manufacturer recovery media or Windows 11 installation media as a last resort; a reinstall from installation media erases the device.

What error 0x8007045D means

Microsoft’s Windows error-code reference defines 0x0000045D as “The request could not be performed because of an I/O device error.” Windows commonly renders the same condition with the high bit set as 0x8007045D. It is an I/O-path error, not a unique System Restore bug.

An input/output request is any read or write Windows makes to storage or another device. During a restore, Windows may be reading the restore point, writing changed system files, or accessing a volume needed to boot. A problem anywhere along that path can produce the same code:

  • recovery media that cannot be read reliably.

Because several faults map to one code, replacing a cable or rerunning Restore without testing the volume may only make the symptom recur. Work from the least destructive checks toward recovery media and a reinstall.

Choose the least-destructive recovery path

Option Data preservation Boot requirement Scope Hardware diagnosis
CHKDSK scan or repair Back up first; /f /r changes the file system and can stress a failing disk Elevated Command Prompt; WinRE can be used when Windows will not start Checks and repairs file-system and media errors on one volume Can report file-system or media problems, but cannot repair a physically failing device
System Restore Generally preserves personal files; rolls back system files and settings WinRE: Troubleshoot > Advanced options > System Restore Returns Windows system files and settings to a selected restore point Does not identify or fix a failing cable, port, controller, or disk
Manufacturer recovery media Follow the manufacturer’s data-erasure warning and back up first Boot from the manufacturer’s recovery media Restores the computer using hardware-specific recovery software Useful when device-specific drivers or recovery tools are required
Windows 11 installation media Reinstalling from installation media removes everything from the device Create the USB on another working PC, then boot the affected PC from it Replaces Windows rather than merely rolling back settings Not a repair for physically failing storage or another defective I/O component

1. Protect your data before changing anything

If Windows still starts, copy documents, saves, photos, browser data, and any other irreplaceable files to a separate drive or a cloud location. Use a destination that is not the volume showing the error. Verify that important files open from the backup before proceeding.

System Restore normally leaves personal files alone, but it changes system files and settings. CHKDSK repair operations write to the file system, and a reset or reinstall can remove data. A backup is therefore the first step, not an optional precaution.

If the drive repeatedly disconnects, makes unusual noises, reports read errors, or causes Explorer to freeze, stop repeated repair attempts and prioritize copying data. Every additional write can make recovery harder on a deteriorating device.

2. Remove avoidable devices and connections

  1. Connect the computer to AC power. A shutdown during storage repair or System Restore can leave Windows unusable.
  2. Disconnect nonessential USB devices, hubs, card readers, and external drives. Leave connected only the keyboard, mouse, display, and the storage needed for the repair.
  3. If the affected volume is external, connect it directly to a known-good USB port instead of through a hub or dock. Try a different known-good cable when one is available.
  4. For an internal drive, power the PC down completely before checking that the drive’s data and power connections are firmly seated. Do not open a device that you are not qualified to service.
  5. Retry the operation once. If the error follows one external drive, enclosure, cable, or port, stop writing to that device and treat it as the suspect part of the I/O path.

These checks do not prove that a disk is healthy; they simply remove common connection variables before you run a repair that writes to the volume.

3. Identify the correct volume

Note the drive letter named in the error, if one is shown. In a running Windows session, confirm it in File Explorer or Disk Management before opening Command Prompt. Do not substitute the Windows drive by guesswork.

Drive letters can change in WinRE. A letter that is C: in normal Windows might be different in recovery. Confirm the volume by its label, size, and folder contents before running CHKDSK, and recheck the target if you return to WinRE later. Running a repair against the wrong volume can damage a healthy disk while leaving the real problem untouched.

4. Run a read-only CHKDSK scan first

A read-only scan gives you information without immediately changing the file system. Start it while Windows is running if possible:

  1. Open Start and type Command Prompt.
  2. Right-click Command Prompt and choose Run as administrator.
  3. Run the command below, replacing X: with the confirmed affected drive letter:
    chkdsk X: /scan
  4. Let the scan finish and record whether Windows reports file-system errors, unreadable areas, or no problems.

The scan may take time, particularly on a large or busy volume. Do not close the window or power off while it is running. A clean result does not rule out an intermittent cable, controller, enclosure, or hardware failure; it only means this scan did not find a problem it could report.

5. Repair the volume with CHKDSK

Use a repair command only after your files are backed up and you have confirmed the drive letter. Microsoft Q&A guidance for this specific error commonly recommends CHKDSK with /R; that is community troubleshooting guidance, not a promise that the command will fix every cause of 0x8007045D.

  1. Open an elevated Command Prompt again.
  2. Run:
    chkdsk X: /f /r
  3. If Windows says the volume is in use and asks to schedule the check for the next restart, type Y, press Enter, and restart when you are ready.
  4. Allow the check to complete. Do not interrupt it, even if the percentage appears to pause.
  5. Read the final report. Errors that are corrected point to file-system or media damage; repeated unreadable areas, disconnects, or a failure to complete indicate that the physical I/O path may still be unreliable.

/f requests file-system corrections. /r looks for unreadable sectors and attempts to recover readable information, so it can take a long time and place additional stress on a failing disk. If the drive is deteriorating, stop after protecting data and arrange replacement or professional recovery rather than repeating the command.

6. Retry System Restore in Windows Recovery Environment

System Restore rolls back system files and settings to a selected restore point; Microsoft states that it does not affect personal files. It is therefore the next step after the volume and connection checks, provided the disk can be read reliably.

When Windows still starts

  1. Open Settings > System > Recovery.
  2. Under Advanced startup, select Restart now and confirm the restart. You can also hold Shift while selecting Restart from the Start power menu.
  3. In WinRE, choose Troubleshoot > Advanced options > System Restore.
  4. Select the affected Windows installation if prompted, then choose a restore point dated before the problem began.
  5. Review the restore point and start the operation. Keep the computer connected to AC power and do not turn it off while the restore is running.

When Windows will not start

Allow Windows to enter Automatic Repair after failed starts, or boot from recovery media and choose the option that opens the computer’s recovery tools. Then select Troubleshoot > Advanced options > System Restore and follow the same restore-point prompts.

If BitLocker protects the system volume, WinRE may request the BitLocker recovery key before it can access the drive. Retrieve that key before starting the restore; without it, the recovery environment may not be able to unlock the volume.

7. If System Restore still returns 0x8007045D

Separate a restore-point problem from a hardware problem

Try a different restore point if more than one is available. If every point fails with the same I/O code, the common factor is more likely to be the volume or the path to it than one corrupted restore point. Recheck the drive letter in WinRE, run the read-only scan against the correct volume, and inspect the cable, port, and enclosure again.

Test the physical path, not just Windows

  • For an external drive, test a direct connection to another known-good port and, if possible, another known-good cable or enclosure.
  • For an internal drive, have a qualified technician check the SATA data cable, power lead, port, and storage controller.
  • If the error follows the drive across ports or computers, stop repeated writes. The disk or its enclosure is a stronger suspect than the original Windows installation.
  • If only one port or cable produces the error, replace that component before attempting another restore.

CHKDSK can diagnose file-system or media problems, but it cannot repair a physically failing drive, controller, or cable. A successful CHKDSK run is not a health certificate for the hardware.

Check whether recovery itself is the failing component

If normal WinRE tools cannot read the restore point, the recovery partition or the storage path used to reach it may be damaged. Booting from separate recovery media removes the internal recovery environment as a variable, but it does not make a failing disk safe to use. Back up first whenever the files remain accessible.

8. Use recovery media only after verifying backups

Microsoft lists manufacturer recovery media and Windows installation media as fallback options when built-in recovery cannot complete. Prefer manufacturer media when the computer needs hardware-specific drivers or custom recovery software.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Create Windows 11 installation media

  1. On another working PC, create a bootable Windows 11 USB using Microsoft’s installation-media process.
  2. Connect the USB to the affected PC and boot from it using the computer’s boot-device menu.
  3. Choose the recovery option, when offered, to access troubleshooting tools before selecting a reinstall.
  4. If you proceed to reinstall, verify one final time that the required files are backed up and that the correct disk is selected.

Installation media is a full replacement path, not an in-place equivalent of System Restore. Microsoft Support warns that reinstalling Windows from installation media removes everything from the device. A reinstall also cannot cure a physically failing disk, bad cable, port, enclosure, or controller; replace or repair the faulty hardware first.

Common mistakes that make 0x8007045D worse

  • Running CHKDSK on the wrong letter: confirm the volume in the environment where the command runs, especially WinRE.
  • Skipping the backup: repairs, resets, and reinstalls can change or erase data.
  • Repeating writes to a disappearing drive: disconnects and read errors are reasons to protect data, not to keep retrying.
  • Using a hub or questionable cable: connect the drive directly to a known-good port first.
  • Powering off during restore: an interrupted restore can leave the computer unusable.
  • Assuming a clean scan proves hardware health: intermittent electrical and controller faults may not appear in one scan.
  • Jumping straight to reinstall: reinstalling erases the device and does not repair defective storage hardware.

When the repair is complete

After a successful restore or CHKDSK repair, reconnect external devices one at a time and test the operation that originally failed. Keep the backup until the computer has remained stable through several restarts and you have confirmed that important files open normally. If the same device continues to trigger I/O errors, plan to replace the suspect cable, enclosure, or drive rather than relying on another restore point.

In short, 0x8007045D identifies an I/O failure path, not a single Windows 11 defect. Data protection, connection checks, a correctly targeted CHKDSK scan, and System Restore through WinRE are the safest sequence. Recovery media is the fallback, and a Windows installation USB should be treated as an erase-and-reinstall option rather than a routine repair.

It runs on Windows 11, 10, 8, and 7; its free trial is offered by category for at least two days before a paid subscription. Results vary, it complements an antivirus rather than replacing one, and no tool repairs every crash.
