If it is not there, match the exact error to the file format, PHP size limits, directory permissions, image-processing libraries, browser or plugin interference, or rewrite rules left broken after a migration. The sequence below isolates each cause without weakening your site’s security.

Start with the fastest, safest checks
- Capture the exact wording. Copy the complete notice, including whether it says HTTP error, unexpected response, server cannot process the image, or image transcoding error. Different messages point to different layers of the upload process.
- Reload the Media Library. Open Media → Library, refresh the page, and search for the filename. If the item is present, do not upload it repeatedly; edit its title or attachment details and continue.
- Test one small, ordinary image. Use a file you know opens correctly and convert it to JPEG or PNG. Upload only that file, preferably in a private browser window. A successful small test distinguishes a bad file from a site-wide problem.
- Record the timing. Note whether the failure began after a host move, domain change, plugin or theme update, PHP change, or browser update. A migration-related 404 follows a different repair path from a file that exceeds a PHP limit.
Do not begin by changing every permission to 777 or by raising every server limit. Those changes can create a security problem while leaving the actual fault untouched.

What each WordPress upload error usually means

The Block Editor documentation lists JPEG, PNG, WebP, AVIF, GIF, and HEIC as supported client-side formats. “Supported” does not guarantee that every server image library can decode every file or that a particular HEIC/AVIF variant is valid. A small JPEG or PNG is the most useful control test.
Run the complete diagnosis in order
1. Prove whether the file is actually missing
An error toast is not proof that no attachment was created. Refresh the library, switch between grid and list view, and search by the original filename. If it appears, open the attachment and check that its preview and file URL work. Retrying can create duplicate attachments and waste storage. If it does not appear, continue with the same filename, size, and exact error recorded.

2. Eliminate a bad or unsupported file
Open the source image outside WordPress. If it will not open, export it again from an image editor. For diagnosis, remove unusual metadata, use RGB or sRGB, and choose a modest pixel dimension; the objective is to test the upload path, not to preserve a damaged original. Save a copy as JPEG or PNG and upload only that copy.
Test files one at a time. If the converted test succeeds but the original fails, the site is probably healthy and the original’s encoding, corruption, or unsupported feature is the cause. If even a tiny JPEG fails, move to server and request diagnostics rather than repeatedly converting files.

In the dashboard, open Tools → Site Health → Info. Expand the server and media-related sections and record the values exactly as shown. Site Health is safer than guessing at a configuration file because it reports what PHP and WordPress are actually using for this request.
file_uploads: it must be enabled for PHP uploads to work.- Maximum post size: the largest total HTTP POST body PHP accepts.
- Maximum upload-file size: the largest individual file PHP accepts.
- Maximum effective file size: the smaller of those two limits. This is the practical ceiling for one upload, not a universal WordPress recommendation.
- Maximum files per request: relevant when a batch upload fails while a single file works.
- GD and Imagick: note whether each extension is available, its version, and the formats it reports. WordPress needs a usable image editor to create thumbnails and other sizes.
- Filesystem status: check whether the WordPress installation,
wp-content, anduploadsdirectories are reported writable.
If file_uploads is disabled, the effective size is lower than your file, an extension is missing, or a directory is not writable, the setting may be controlled by the host. Do not edit random PHP files until you know which value is failing.

4. Compare the file with the effective limit
Check the file’s size in your operating system, then compare it with Site Health’s maximum effective file size. The effective value is the smaller PHP post_max_size and upload_max_filesize setting, so increasing only one value cannot solve a conflict with the other. Remember that the post limit includes the multipart request overhead and any additional fields, while the upload limit applies to the file itself.
A file below the WordPress value can still fail if a reverse proxy, web-application firewall, host-level rule, timeout, memory ceiling, or temporary directory is more restrictive. Give support the filename, byte size, timestamp, exact notice, and the Site Health values instead of asking for an arbitrary limit.

5. Correct the uploads directory with least privilege
WordPress documentation notes that media uploads may require correct permissions on wp-content. The web-server process must be able to create files and directories under the relevant year/month path, and the directory must have the correct owner and group for your hosting arrangement.
- Identify the exact uploads path, normally
wp-content/uploads, and the account used by PHP or the web server. On managed hosting, ask support for both rather than guessing. - Check ownership and writability of that directory and its existing year/month folders. A migration can leave files owned by an old system account.
- Change only the affected path to the host’s documented safe ownership and permission settings. Apply the same correction to newly created subdirectories if necessary.
- Retry one small image and confirm that WordPress can create its attachment metadata and thumbnail.
Never make the entire installation world-writable as a shortcut. WordPress Developer Resources warns that 777 permissions can let an attacker upload malicious content or modify executable files, potentially exposing the database and passwords. If you cannot establish the correct web-process owner and group, stop and ask the host to repair them.

6. Check image libraries, temporary storage, and resource ceilings
GD or Imagick may be installed but unable to handle the particular format, or the server may run out of memory or temporary disk space while creating thumbnails. A large, high-resolution image can therefore fail even when its compressed file size is under the upload limit.
Use the GD and Imagick information in Site Health to see which editor is available and which formats it reports. Compare a small JPEG with the failing format. If JPEG works but HEIC, AVIF, or another format does not, the server library may lack that decoder; converting the file is the immediate workaround. If every format fails, ask the host to inspect PHP memory, temporary upload storage, process timeouts, and the image-library error log.

7. Separate browser, plugin, theme, and security-layer faults
Upload from another current browser or a private window. If it succeeds there, disable extensions in the failing browser and clear its site data before testing again. A browser extension, stale authentication cookie, or cached JavaScript bundle can produce an “unexpected response” without any server-side change.

If the problem reproduces across browsers, isolate WordPress code safely. Use a staging copy or a maintenance-safe troubleshooting workflow to deactivate plugins one at a time, beginning with security, media-optimization, caching, and firewall plugins. Switch temporarily to a standard theme in staging if a theme adds custom upload handling. Re-test after each change and record the result; do not leave unrelated production plugins disabled.
Open the browser developer tools and inspect the failed admin or REST request. Record its HTTP status, response body, and endpoint. A 403 suggests a security rule or capability issue, a 413 points toward request-size enforcement, and a 500 indicates a server-side failure; the exact response is more useful to support than a screenshot of the toast.
Repair uploads that broke after a migration
Migration failures often look like upload failures because the attachment record exists while the generated URL returns 404, or because a request ends on a white screen. First check whether old files are physically present in wp-content/uploads and whether the attachment appears in Media Library.
Flush WordPress rewrite rules
- Go to Settings → Permalinks.
- Leave the existing permalink structure selected.
- Click Save Changes once to regenerate rewrite rules.
- Open an affected image URL in a new tab and test a fresh upload.
Saving the page flushes WordPress’ rewrite rules without requiring a new URL structure. If image URLs still return 404, ask the host to verify Apache mod_rewrite support and the generated .htaccess file. A missing, unreadable, or misplaced .htaccess can prevent requests from reaching the uploads directory even when filesystem permissions look correct.

Check the site and WordPress URLs after a domain move, including HTTPS and any subdirectory. A mismatched URL can make existing media appear missing while the files remain on disk. Have the migration provider correct serialized database URLs with a WordPress-aware procedure; do not run blind search-and-replace commands against serialized data.
When the host must make the change
Escalate when Site Health shows a disabled setting, an insufficient effective limit, missing GD/Imagick support, unwritable directories you cannot safely re-own, or a host-level 403, 413, timeout, or 500 response. Hosting support can inspect PHP-FPM or Apache/Nginx logs, temporary storage, process limits, firewall rules, and filesystem ownership that WordPress cannot change from the dashboard.
Send a compact diagnostic bundle:
- Site URL and the affected administrator account (never send a password).
- Exact error text, timestamp with timezone, and whether the file appears after reloading Media Library.
- Filename, extension, pixel dimensions, and byte size of both a failing file and a small known-good test.
- Site Health values for
file_uploads, post size, upload size, effective size, files per request, GD/Imagick, and directory writability. - The browser, whether a private window or second browser changes the result, and the failed request’s status code.
- Whether the issue started immediately after migration, a PHP change, or a plugin/theme update.
Enable debug logging only for the shortest practical diagnostic window and keep errors out of public pages. Logs can contain paths, usernames, request data, and other sensitive information. After collecting the evidence, disable public display and remove temporary diagnostic files.
Prevent the next media-upload outage
- Keep a small JPEG or PNG control image available for testing.
- Document the host’s approved PHP limits, image extensions, ownership, and permission model before a migration.
- After moving a site, verify one existing image URL, save Permalinks, and upload a new image before declaring the move complete.
- Use supported, web-sized formats for routine publishing; retain originals outside WordPress when they are unusually large or use specialist codecs.
- Update plugins, themes, and PHP in staging first, then test one upload and thumbnail generation.
- Back up the database and
wp-content/uploadsbefore changing permissions or migration URLs. - Never solve an upload error by making every file or directory 777.

This process narrows the fault from a misleading upload notice to a specific file, request, PHP setting, directory, image library, browser layer, or rewrite configuration. Make the smallest correction that restores uploads, then retest with the same known-good file.
