It briefly pins Mohg, Lord of Blood, to the ground, but only works up to twice per fight and only during phase one.
Where to find Mohg’s Shackle
Search the bottom of the large, pipe-filled room in the Subterranean Shunning-Grounds beneath Leyndell. Two Giant Crayfish guard the item, so be ready to deal with them or move past them to reach it. The item reference lists this location at Elden Ring Wiki: Mohg’s Shackle.
How to reach Mohg, Lord of Blood
The Shackle’s location and Mohg, Lord of Blood’s arena are separate places: the item is under Leyndell, while the boss is at Mohgwyn Palace. Bandai Namco lists two ways to reach the palace: use the sending gate in the Consecrated Snowfield or progress White Mask Varré’s questline to obtain the Pureblood Knight’s Medal. How Mohg’s Shackle works in the Lord of Blood fight
Use the Shackle during phase one to briefly restrict Mohg’s movement. It can bind him at most twice per fight, so avoid spending both uses early if you want to save one for a later opening. PlayStation’s official Elden Ring page recommends using it up to two times in the first half of the two-phase fight.

The Shackle does not deal damage, cancel Mohg’s phase transition, or interrupt the Bloodboon Ritual. According to the community-maintained item reference, its binding effect stops working once Mohg enters phase two. Use it to create a brief opening in the first half, not as a way to prevent the transition.
Does it work on Mohg, the Omen?
The community-maintained item reference also says Mohg’s Shackle works against Mohg, the Omen. That is a separate encounter from Mohg, Lord of Blood, so do not confuse the Shackle’s location beneath Leyndell with the Lord of Blood’s arena at Mohgwyn Palace.
