To reach the Cultist Lair in Fisch, travel to Terrapin Hideaway, find the entrance lever behind two pine trees on a foggy night, activate it, and enter the portal. At the gate beyond the portal, get past the guards without being spotted.

Go to Terrapin Hideaway. Look behind two pine trees for the entrance lever. The community guides describe the lever as appearing on foggy nights, so if it is not there, return when the weather is foggy. The Cultist Lair guide and TheGamer’s route guide both describe this approach.

Activate the lever when it appears, then walk into the portal it opens.
Get through the gate
After going through the portal, approach the gate carefully. Guards are watching the entrance; avoid being spotted as you make your way into the lair. The guides identify avoiding the guards as the way through, but do not specify a particular stealth mechanic or exact guard pattern.

What to know about the location
The Cultist Lair is a sub-location in Fisch. Its community guide says it was added in version 1.57. The route details here come from community guides, not official game documentation.
Coordinates are incomplete
A location listing gives Y: 155 and Z: 2000, but marks X as unknown. Those values do not make a complete GPS coordinate, so use Terrapin Hideaway and the two pine trees as your landmarks rather than relying on a full coordinate.
