What you need for a custom Operation game
The essentials are a stable board, matching openings and pieces, an extraction tool, and a rule that tells players when a removal succeeds or fails. Foam board, wire, basic electronic components, a buzzer and an LED are named in an Instructables project summary; treat these as a starting point, not a complete or verified bill of materials. - Foam board for a lightweight prototype, with backing or reinforcement if it flexes.
- Lightweight pieces sized to fit their openings.
- Tweezers or another narrow extraction tool.
- Optional: wire, a buzzer, an LED and suitable basic electronic components.
- Paper or cards for naming pieces and assigning turns or targets.
No exact board dimensions, cavity sizes or homemade circuit schematic are established by the project reference. Choose dimensions by testing your own pieces and tool rather than relying on a supposed standard.
Plan the board, cavities and pieces
Choose a theme and draw the patient
Pick a character, creature, robot or other subject that can be divided into distinct removable parts. Draw or print it on a board that stays steady during play. Mark each cavity clearly, leaving enough surrounding margin for the extraction tool to reach the piece without the board breaking or bending.

Make pieces that sit flat and lift cleanly
Cut lightweight shapes that fit their matching spaces. Each piece should lie flat and come out without catching. Hasbro’s Star Wars edition manual advises that pieces lie flat in their matching spaces and mentions smoothing excess plastic from molded pieces with an emery board or sandpaper. Name each piece to match the theme. Distinctive names make it easier to call out targets and create cards or a simple scoring sheet.

Build the board and choose an extraction tool
Cut openings in the board to match the pieces, then test each opening before decorating or attaching electronics. Foam board is identified in the project summary as a material used for an Operation-style build; reinforce it if handling makes it flex. Use tweezers sized so players can reach into the openings while keeping a steady grip. Hasbro’s classic game reference includes tweezers and removable plastic ailments. Try several piece-and-cavity combinations before settling on the final layout. A larger piece with more clearance is easier to grasp; tighter clearance makes the task harder. These are design choices, not published measurements or tested difficulty ratings.

Decide whether to add a buzzer and light
Electronic version
The familiar electronic penalty happens when the extraction tool touches the conductive edge around a cavity: the contact triggers sound and light. Hasbro describes that mechanic in the Star Wars edition manual, and the Instructables project summary likewise describes tweezers touching a conductive cavity edge to activate a buzzer and LED. The precise parts, wiring and circuit values are not established by those references.
If you add electronics, plan the contact surface and tool as part of a small prototype first. Test whether contact reliably triggers the signal and whether the piece can be removed without an accidental warning. Use an appropriately low-voltage, battery-powered approach and do not connect a homemade game circuit to mains power. The cited references do not provide a safety review or a complete wiring guide.

Non-electronic version
You can leave out the circuit, but players still need an unmistakable way to judge a miss. Agree on a referee call or use a timer as a house rule. This is a simple alternative rule, not a sourced design tested in the references.
Test the prototype before decorating it
- Place every piece in its matching opening and check that it lies flat.
- Remove and replace each piece several times with the chosen tool; adjust any opening that jams or makes a clean lift impractical.
- For an electronic build, check each cavity’s contact response and make sure the signal occurs when the tool touches the boundary, not during an ordinary removal.
- Have players try the board and adjust piece size, spacing or tool choice if the task is too easy or difficult for your intended group.
- Reinforce the board if it bends during these checks, then add labels and decoration.
Write simple rules and start playing
Hasbro describes a game loop using doctor cards, matching removable ailments and payments for successful removals, with the highest total winning. For a simpler custom version, use one point per clean extraction. Hasbro’s classic Operation game page

- Take turns drawing a card or choosing a named piece as the target.
- Attempt to lift that piece without touching the cavity boundary.
- A clean removal earns one point; contact with the edge ends that player’s attempt.
- Continue until all pieces are removed, then count points. The highest score wins.
To echo the official game’s variable rewards, assign different point values to targets or make target cards, but agree on those values before play.
Adjust the design for your players
- For easier play: use larger pieces, more clearance around them and a tool that fits the player comfortably.
- For greater dexterity challenge: reduce clearance gradually, checking that pieces still come out without jamming.
- For quick games: award one point per successful removal and end when the board is empty.
- For a closer nod to the classic: use target cards and different rewards, following the doctor-card and payment idea Hasbro describes.
Hasbro’s classic US product page lists a gameboard, tweezers, 12 plastic ailments and a guide, and says that version requires two AA batteries and adult assembly. Its contents and age guidance may differ across editions and regions. The Star Wars edition manual specifies four AAA batteries, illustrating why battery details should be checked against the particular edition rather than generalized to every Operation game.
