Return to running after a hamstring strain through individualized, progressive rehabilitation—not a fixed number of rest days. Increase running demands in response to symptoms and capacity, and treat sprinting as a later step than easy running. The available guidance does not establish a universal recovery timeline, pain threshold, or running schedule.
What to do after a suspected hamstring strain
Posterior-thigh pain can have causes other than a hamstring strain, and the guidance here focuses on athletic hamstring injuries rather than diagnosis. If you are unsure what is injured, symptoms are severe or persistent, or you are uncertain how to progress, seek assessment from a qualified health professional.
For a confirmed or suspected strain, rehabilitation should be tailored to the person, the hamstring muscle involved, the injury’s type and severity, and the demands of the sport or activity. The 2023 London International Hamstring Injury Consensus group reported 89.8% agreement on this individualized approach.
Build rehabilitation around progressive activity
Rest alone is not the full rehabilitation plan. The 2022 clinical practice guideline for hamstring strain injury in athletes supports strengthening, eccentric training, functional work, and progressive running. Exercise choice and loading need to suit the individual; the guideline does not specifically endorse resistance bands or a particular product.

Progress according to how the hamstring responds to activity, including symptoms during and after loading, and whether capacity is improving. The 2023 consensus allows that some rehabilitation activities may involve mild pain, but experts did not agree on a universal pain threshold or optimal loading dose. Do not convert that into a rule that pain should be ignored or into a single pass/fail number.
Return to running in stages
Running is part of rehabilitation, not merely a test to attempt after rehabilitation is finished. In the 2023 consensus, 98.4% of the panel agreed that running and sprinting are key components of hamstring rehabilitation. That is agreement among experts, not a treatment-effect figure or a promise about an individual’s recovery.

- Establish readiness for running with a clinician when needed. Consider symptoms, current capacity, injury context, and response to earlier loading rather than elapsed time alone.
- Reintroduce running progressively. Increase exposure in a way that reflects the runner’s response. The sources do not prescribe one universal distance, pace, frequency, or increase for everyone.
- Build toward faster running. Faster running places greater demands on the hamstring than easy running, so it belongs later in the progression. Reassess symptoms and response as demands rise.
- Progress to sprinting only as a later task. The consensus supports pain-free sprinting and progression to full sprinting before return to sport. It reported 85.5% agreement for pain-free criteria for sprinting, but did not establish a universal speed, volume, or progression threshold.
Why an easy run does not clear you for sport
Completing an easy run is one milestone; it does not establish readiness for maximal sprinting, full training, or competition. Return decisions should reflect the actual demands of the person’s sport, including the faster running and sprinting it requires.
A single strength percentage, flexibility measure, special test, or time estimate is not established by this consensus as a universal clearance rule. The panel did not reach consensus on flexibility benchmarks (40% agreement) or strength benchmarks (66.1%). A clinician can interpret relevant testing alongside symptoms, capacity, training demands, and the response to progressively harder work.

A previous hamstring strain also matters. The 2022 clinical practice guideline identifies prior injury as a reinjury risk factor and recommends considering injury history during return-to-play progression.

There is no single timeline in the cited guidance that can safely answer this for every runner. Injury type and severity, the muscle involved, activity demands, prior injury, and response to rehabilitation all affect progression. The 2023 consensus identifies loading dose, progression timing, and the best monitoring and testing measures as areas where more evidence is needed. A calendar estimate should not replace an individualized assessment.
