First, turn off Nudity Censor in Settings → Gameplay from the main menu. If V still wears underwear during normal gameplay, the available removal options described here are PC mods—not a guaranteed effect of that setting. Console mod support is not established by the sources cited below.
Try the built-in nudity setting first

Check the result in gameplay. The setting’s location is documented by a third-party page posted in 2021, so labels or menu placement may vary by game version or language. What the other listed mod does—and does not do
Nevernude underwear replaces is an appearance replacement, not an underwear-removal tool. Its page says the nudity setting should be off and notes that, with the setting on, the replacement result can be covered as clothing. Nexus Mods listing

Mod support and console limits
CD PROJEKT RED’s modding support page says community-created mods are not maintained by the studio and can affect support for a modified game. Its REDmod documentation covers PC mod loading, including support for legacy archive mods; REDmod cannot manage the load order of those legacy mods.
The underwear-removal and wardrobe options described above are PC downloads. The cited sources do not establish a console mod route, so do not follow these PC installation steps on PlayStation or Xbox. For consoles, the built-in nudity setting is the only option supported by these sources.
