To start Karlach’s romance, recruit her, build her approval to at least 20, then talk to her at the Act One Tiefling celebration and accept her offer to spend time together. For the clearest route, speak with Karlach before starting another companion’s romance scene that night. Dammon’s engine upgrade can help with approval, but handing over Infernal Iron alone does not trigger the romance.
Start Karlach’s romance at the Act One celebration
- Recruit Karlach. Bring her into your party so you can build approval through your choices.
- Reach at least 20 approval. Take actions she approves of; letting Dammon perform her first engine upgrade can provide an approval boost if you have Infernal Iron. Approval changes may fail to register because of bugs, mods, or because Karlach is already at maximum approval. See Karlach’s approval opportunities.
- Resolve the Emerald Grove and Tiefling situation and proceed to the camp celebration.
- Talk to Karlach before initiating another companion’s celebration scene if you want her as your first choice. Choose the responses that reciprocate her interest and agree to spend time with her.
The consulted romance guide gives 20 approval as the threshold for accepting Karlach as a first choice, provided you have not initiated another romance scene during the celebration. It describes a second-choice route as possible with at least 20 approval and no more than one other active romance. These are guide-reported conditions, not an official Larian guarantee. Choose the celebration route that fits your situation

What Dammon’s engine repair does—and does not do
Dammon’s first engine upgrade is useful for Karlach’s quest and can raise her approval, but it is not by itself the romance trigger. The consulted guide identifies the Tiefling celebration as the current initiation point. Treat the repair as progress that can help you meet the approval condition, not as a substitute for the celebration conversation. Approval reference.
Continue the relationship in Act Two
After the celebration scene, keep progressing Karlach’s infernal-engine quest. The guide says her Act Two relationship scene becomes available after you reach Last Light Inn and Dammon repairs the engine. Take a long rest when no other camp events are queued to give the scene a chance to occur. Avoid partnering with someone else before this scene: the guide warns that doing so ends Karlach’s relationship, and leaving Act Two without the scene may close the progression path. If Karlach turns you down or the scene does not appear
- Check approval. The guide says Karlach declines at 19 approval or lower in the celebration route.
- Check the order of scenes. For the first-choice route, do not start another companion romance scene during the celebration. The guide also lists prior romance states among reasons she may reject the approach.
- Check whether approval is registering. Bugs, mods, or already-maxed approval can prevent a displayed change.
- For Act Two, check quest and camp timing. Reach Last Light Inn, have Dammon repair the engine, then rest when no other event is queued.
- Account for patch and mod differences. Larian’s Patch 2 notes describe added Karlach moments in Acts One and Two and an ending scene for her Avernus decision. The Patch 7 materials document several Karlach scene and dialogue fixes. Those notes do not establish that every reported romance-start trigger issue is fixed.
