Craft the Heart of the Elements
The Heart of the Elements is a Hardmode accessory. Its listed recipe is:

- 1 Elemental in a Bottle
- 1 Oasis Elemental in a Bottle
- 1 Pearl of Enthrallment
- 1 Eye of the Storm
- 1 Rose Stone
- 6 Stardust Fragments

Equip it to summon the five elementals
Put the crafted Heart of the Elements in an accessory slot. Equipping it is the listed action that summons the group; crafting the accessory alone is not described as summoning them. The five are the Brimstone, Sand, Cloud, Water, and Oasis Elementals. The Oasis Elemental heals you, while the other four attack nearby enemies. The Water Elemental floats over the player, according to the [Calamity minions reference].

If you mean the Brimstone Elemental boss
The friendly Brimstone Elemental minion from the Heart of the Elements is different from the Brimstone Elemental boss encounter. To summon the boss, use a Charred Idol in the Brimstone Crag. The Charred Idol is not consumable; its listed recipe is 5 Unholy Cores, 7 Essence of Havoc, and 5 Souls of Night at a Hellforge. The Elemental in a Bottle is a Hardmode accessory dropped by Sand Elementals and is one of the Heart’s ingredients; its [item page] identifies its role in the recipe. The cited item information establishes the names of the Heart’s other ingredients, but not acquisition instructions for each one, so their drop sources and progression requirements are not listed here.
The Heart of the Elements history page records a Stardust Fragment recipe-count change in version 2.1.0 and a change to elemental appearance when visibility is disabled in version 2.2.2.
