To unlock the Red Death skill tree, complete the Cuanacht side quest Perinde Ac Cadaver. Start by speaking with Ignatius at the hospital, follow the quest to The One at Toll House, deliver the Marks of the Plague, then finish the Asylum sequence. After defeating the Plaguewraith, speak with Prophet Cai to complete the quest and receive the tree.

How to start Perinde Ac Cadaver
- Go to the hospital in Cuanacht. Speak with Ignatius. The quest reference says to talk to him after accepting the gift of hollowness; a video-guide description phrases the dialogue prompt as accepting a mark. If the quest does not begin, check that you have accepted the relevant mark dialogue. Quest reference; video-guide description
- Find The One at Toll House. Follow the quest marker to Toll House and use the elevator to reach the lower level. Speak with The One.
- Deliver the Marks of the Plague. The quest walkthrough directs you to speak with Ignatius and Úna and hand over the marks, then return to The One with the last mark.
- Finish the Asylum sequence. Continue through the Asylum and defeat the Plaguewraith. Then speak with Prophet Cai to finish the quest and unlock the Red Death tree.
Before giving Úna a Mark of the Plague
A 2025 walkthrough warns that giving Úna a Mark of the Plague can block completion of her separate quest, I’ll Be Waiting. If you want to preserve that quest, finish it before handing her the mark. What the Red Death tree contains
The available skills-page extract describes a set of skills built around Lament stacks: Cry In Pain grants Lament stacks as you lose health, Long Regret delays their decay, and Give In And Give Up increases the maximum number you can hold. Quest rewards
The quest reference lists the Red Death skill tree, Prophet’s Set, and 2,400 XP as rewards for Perinde Ac Cadaver. The XP figure is the listed in-game quest reward. Quest reference
Red Death in the game versus the tabletop expansion
