To use the Shark Fin, defeat the Great White One on Whakapanu Island during Act 4, collect the fin, and hand it to Kaimana to finish the Tribal Medicine quest. Where you find her depends on how far you have progressed in Act 4: she may be in Ngakanu or aboard the Karui boat in Kingsmarch. The quest reward is the one part you should confirm in your own game, because the sources available for this guide disagree about it.
What you need before you start
- Act 4 progress. The Great White One fight and the Tribal Medicine handoff both take place in Act 4.
- Access to Whakapanu Island, where the Great White One is found.
- The Shark Fin, which drops from the Great White One and is the item the quest asks for.
Step by step: from the fin to the finished quest
- Defeat the Great White One on Whakapanu Island and collect the Shark Fin. Picking up the fin is tied to the Tribal Medicine quest, so you will not need to look for a separate quest-start trigger.
- Locate Kaimana. Go to Ngakanu first. If she is not there at your current story point, use the location table below.
- Speak to Kaimana and give her the Shark Fin. The hand-in is the completion step described in the quest reference.
- Read the final dialogue and reward text in-game before you accept anything, and note what it offers. Do not rely on a guide’s description of the reward; see the reward section below.
Finding Kaimana: Ngakanu or the Karui boat
Kaimana is not in one fixed place for every player. Her location depends on how far you have progressed through Act 4, so the table below gives both locations and when each applies.
| Location | When to check | Notes |
|---|---|---|
| Ngakanu | Check first, while you are still at the earlier Act 4 stage | Described by the community quest wiki as one of her two possible locations. |
| Karui boat, Kingsmarch | After further Act 4 progress, if she is no longer in Ngakanu | Described by the community quest wiki and by PC Gamer, whose guide says her location varies with Act 4 progress. |
If she is in neither place, your story progress is probably not where the guides assume. Continue through Act 4 and check both locations again rather than searching the map for a different NPC.
The reward: what is confirmed and what is not
The reward is the least settled part of this quest. The sources we checked describe different outcomes, and none of them is clearly tied to the current game version.

| Source | Date or version | What it says about the reward | How far to rely on it |
|---|---|---|---|
| PC Gamer guide | Dated 2 September 2025 | Describes a choice between a spirit gem, a support gem, or a skill gem. | Reliable only as a record of the game at that date. It predates later changes that may apply now. |
| Community quest wiki | No patch version stated | Describes the hand-in and both Kaimana locations. | Useful for the route. Does not establish a reward or a patch version. |
| FandomWire | Undated in the material found | Claims the reward was changed to permanent defensive passives. | Not supported by the official patch-note material we located. Treat as unconfirmed. |
| PoE2 Hub guide | Labelled as checked against patch 0.5.4b | Makes reward statements. | The available excerpt does not confirm those statements, so the patch label alone is not enough to rely on them. |
| Official Path of Exile forum | Patch notes for 0.4.0 and a 0.5.0 timeline announcement | Does not state a change to the Tribal Medicine reward. | The closest official source we found. It is silent on the reward rather than confirming any version. |
Taken together, the gem choice in the PC Gamer guide is the best-documented description, but it is dated, and the permanent defensive passive claim has no official support in the material we found. Confirm the reward in the live quest dialogue before you choose anything. If the reward you see differs from a guide, assume the guide is out of date.
