Mafia: The Old Country has a clear identity in its 1900s Sicilian setting and Enzo Favara’s climb into the Torrisi crime family. Its weaker point, according to the split critical reception, is the action around that story: stealth, knife fights and gunplay can feel familiar or repetitive rather than defining. Whether it is worth playing depends largely on how much you value a focused crime drama over mechanical variety.
What kind of game is Mafia: The Old Country?

The game’s structure is cinematic and story-led, with stealth, knife fights and firearms alongside horseback travel and period automobiles. That mix supports the period drama, but the official descriptions do not promise a sprawling sandbox. Its identity is closer to a focused crime story than an open-ended world built around player freedom.
Does the setting give the game an identity of its own?
Yes, most clearly in its specific time and place. The Sicilian setting, period detail and family-crime premise give the story a strong frame, while Enzo’s attempt to find his place in the Torrisi family provides a personal through-line. Positive critic summaries have singled out the story, characters, atmosphere and presentation.

- UNCOVER A MERCILESS WORLD OF ORGANIZED CRIME: The rough-hewn beauty of Sicily's rugged countryside is a stark contrast to the grime of urban alleyways—but the treachery and violence of this gangland run just as deep. Rival families and their ruthless leaders wage unending turf wars in the shadows, hidden from the public eye. Trust is a fleeting rarity, and loyalties are worth killing for.
- PLAY A CLASSIC MOB MOVIE: You're the antihero of this thrilling 1900s story, living out every tense moment of Enzo's descent into Sicily’s clandestine criminal underworld. Come face-to-face with a cast of unpredictable allies and cutthroat enemies in this classic crime drama, rich in period-authentic details that will immerse you in this treacherous Mediterranean setting.
- FIGHT TO SURVIVE: Engage in life-or-death combat, be it up close with a blade or from afar with a variety of firearms. Ambush enemies for vicious stealth takedowns, or slice them up in close-quarters duels. If you prefer firearms, wield period-authentic handguns, rifles, and shotguns to overcome any odds and doggedly eliminate the Don's enemies.
- TRAVEL IN AUTHENTIC STYLE: Traverse a wide range of Sicilian environments, from underground crypts and crumbling ruins to bountiful vineyards and ornate opera houses. To reach your next target or make a hurried getaway, you'll need to gallop on horseback across cobbled streets and open fields, or speed down dirt roads in authentic turn-of-the-century automobiles.
That distinction is less pronounced in the moment-to-moment play. More critical summaries describe the gameplay as bland or frustrating, with repetitive encounters, dated stealth and missions that do not make full use of the map’s apparent breadth. The tension is not that the game lacks a recognizable setting; it is that its action may not feel as distinctive as the world around it.
What did critics think?
Metacritic classified critic reception as “Mixed or Average.” When accessed on October 8, 2026, it listed a Metascore of 73 based on 81 critic reviews and a user score of 7.6 from 1,715 ratings. These are a dated snapshot of aggregated scores, not a unanimous verdict or a substitute for reading individual reviews. The range of excerpts reflects the central divide: praise for a tightly told crime story and its period atmosphere, alongside reservations about repetitive or constrained gameplay. Is Mafia: The Old Country worth playing?
It is a stronger fit for players who want a linear, cinematic crime story than for those looking for a mechanically varied sandbox. If Sicilian atmosphere, characters and a focused narrative are the draw, the qualities critics praise may outweigh familiar combat and stealth. If varied encounters and flexible mission design matter most, the critical reservations deserve more weight.
That judgment is about the game’s design and critical reception, not a guarantee of how it runs on a particular system today. 2K’s Store lists PlayStation 5, Xbox Series X|S and PC via Steam. The publisher’s support notes report fixes for progression-blocking companion behavior, subtitles and localization, animations, platform stability, PC performance and a DLSS Frame Generation issue.
