Maternal Staff: Who Provides Maternity and Newborn Care?

Maternal staff is a broad term for professionals who provide or coordinate pregnancy, birth, postpartum, and newborn care. Roles, staffing, and rules vary by setting and location.

“Maternal staff” is not one standardized job title. It generally refers to the professionals who provide or coordinate care during pregnancy, labor, birth, and the postpartum period. The team varies with the care setting, a family’s needs, complications, and local rules.

Who is part of a maternal care team?

Maternity and newborn care may involve clinicians from several specialties as well as educators and community supports. As one Canada-specific example, the Public Health Agency of Canada lists roles that can include:

  • Midwives, family physicians, and obstetricians
  • Neonatologists, pediatricians, nurses, and nurse practitioners
  • Anesthetists
  • Childbirth and parenting educators, doulas, and breastfeeding advisors
  • Social scientists, psychologists, social workers, community supports, and community health or health-promotion workers

This is an illustrative Canadian roster, not a universal list. The agency recommends that professionals collaborate around the needs and preferences of the woman and family. What does maternal staff do across the care continuum?

The work is not limited to delivery. WHO describes midwifery care across prepregnancy, pregnancy, birth, postpartum, and the early weeks of life. WHO’s overview characterizes skilled maternal and newborn professionals as educated, trained, and regulated to applicable standards, with the ability to provide evidence-based, respectful care, recognize complications, and manage or refer them. Which professionals are involved depends on the stage of care and the circumstances. Routine care, a higher-risk pregnancy, a birth requiring anesthesia, and a newborn needing specialized support can call for different combinations of staff. The practical question is not simply how many people are on a roster, but whether the right expertise and backup are available for the patient and setting.

How many nurses are needed for labor and delivery?

There is no single ratio that applies to every patient, facility, or jurisdiction. Canadian service-organization guidance reports postpartum nurse-to-patient ratios ranging from 1:1 to 9:1 depending on complications; that is a reported range, not a recommended target. The same guidance cautions that staffing models from other hospital units may not transfer directly to perinatal care because patient acuity and individual needs vary. That chapter reports AWHONN recommendations for specific healthy mother-newborn situations. These are recommendations cited by Canadian guidance, not universal staffing law or individualized clinical advice:

  • One-to-one nursing for labor with minimal or no pharmacological pain relief or interventions, for patients receiving oxytocin, and for labor complications.
  • Two nurses at vaginal or cesarean birth: one for the mother and one with newborn-resuscitation capability for the baby.
  • Two nurses for up to two hours postpartum: one for the mother and one for the baby, or one for each infant in a multiple birth.

Staffing also depends on experience and skill mix. A tertiary urban hospital and a low-risk birthing center may need different arrangements; rural resource constraints can limit flexibility. In Canada, midwifery regulation and funding also differ among provinces and territories.

Do training and staffing rules vary by location?

Yes. Requirements depend on the jurisdiction and, in some cases, the facility’s designation. Recommendations, designation criteria, and binding rules are not interchangeable; facilities should verify current local requirements before making compliance decisions.

Mississippi: maternal designation rules

Mississippi’s designation document describes an organized maternal program, defined maternal-care privileges, and ongoing staff and team-based education. It assigns the Maternal Medical Director responsibility for staff competency in obstetric emergencies, complications, and resuscitation. It also describes an RN Maternal Program Manager with perinatal experience collaborating on competency, education, and training. These are Mississippi-specific designation rules.   • Stage of care: labor, birth, or postpartum
  • Staff credentials and scope of practice under local rules
  • Nurse assignments and who is responsible for newborn care
  • Availability of obstetric, anesthesia, and neonatal support, including transfer arrangements
  • Required team training and how competencies are documented

These checks help frame a useful discussion, but they do not replace local policy, current regulation, or clinical judgment.

What Canadian workforce figures are available?

The Public Health Agency of Canada reports figures from the Canadian Hospital Maternity Policies and Practices Survey: 72% of births were attended by obstetricians, 25% by family physicians, and 3% by midwives; it also reports that 67% of hospitals regularly scheduled interprofessional meetings. The survey year was not established in the cited guidance passage, so these figures should not be read as current global staffing shares.
