It is most appealing if you want the look and mood of 1970s Italian crime cinema; its Steam user-review snapshot is mixed, so the available evidence does not support calling it a broadly acclaimed must-play.
What is Milanoir about?
Developed by Italo Games and published by Good Shepherd Entertainment, Milanoir follows Piero, a convict released from prison who sets out to find the man who framed him. The official site places the story in a violent Milan shaped by betrayal and greed, and names 1970s Italian crime films—including Caliber 9 and Almost Human—as inspirations. How does Milanoir play?
Steam describes an action mix that includes shooting, choking, sneaking and car chases. It also lists an arena mode and two-player local co-op. Those features suggest more than one kind of encounter, but the store listing alone does not establish how deep or varied those modes feel in practice. Steam store page

If you are choosing primarily for elaborate stealth systems or a substantial co-op campaign, the available feature descriptions do not establish that those are the game’s strengths.
Is Milanoir worth playing?
It is worth considering if you want a pixel-art action game with a specific Italian crime-film influence, and you are comfortable with violence and a modest, mixed Steam user-review snapshot. It is a less certain pick if your decision depends on strong critical consensus, extensive mechanical detail, or a co-op experience whose scope is clearly documented.
