A free scan shows the junk files, broken settings and background clutter dragging Windows down - then fixes them in one click.Free scan · Windows 10 & 11Error 4usqa in classic Outlook is an authentication failure, not automatically a bad-password problem. Microsoft documented a May 2025 incident in which Outlook for Microsoft 365 and Outlook for Microsoft 365 for Mac could show “Something went wrong. [4usqa]” with error CAA2000B. In some tenants, Microsoft Entra ID also returned AADSTS500014 because the service principal for resource ID 40775b29-2688-46b6-a3b5-b256bd04df9f was disabled.
The incident was marked fixed by Microsoft on May 14, 2025. Restart Outlook first. If the error persists, an administrator should verify the Microsoft Information Protection API enterprise application, review Conditional Access and licensing, then use Entra sign-in logs before opening a Microsoft 365 support request.

What the 4usqa error means
The tag 4usqa is displayed by the Outlook sign-in experience when authentication cannot complete. It is a symptom attached to a failed Microsoft 365 client authentication flow; it is not, by itself, a password-validation code. During the documented incident, Outlook could not obtain or refresh the Microsoft Entra ID token it needed to access the mailbox and related Microsoft 365 services.
Users commonly saw a dialog similar to “Something went wrong. [4usqa]” and the code CAA2000B. Some affected sign-ins also produced AADSTS500014, which means the service principal for the requested resource was disabled. When token acquisition fails, Outlook may stop downloading new mail, fail to send, repeatedly prompt for credentials, or open with stale mailbox data.

Microsoft Support said its initial cases pointed to a disabled service principal for resource ID 40775b29-2688-46b6-a3b5-b256bd04df9f. That resource is associated with the Microsoft Information Protection API enterprise application in Microsoft Entra ID.
Which Outlook versions and accounts are in scope?
The documented incident concerned classic Outlook for Microsoft 365 on Windows and Outlook for Microsoft 365 for Mac. It does not establish that every appearance of 4usqa has the same root cause. Outlook.com consumer accounts, personal Microsoft accounts, new Outlook, mobile Outlook, and unrelated Microsoft sign-in errors can involve different services and policies.
A useful distinction is where the failure occurs:
- Web Outlook works, desktop Outlook fails: the account may be valid while the desktop client has a stale token, a blocked application, a disabled enterprise application, or a profile/device-registration problem.
- Web and desktop both fail: investigate the account, license, Conditional Access result, tenant configuration, or a broader service incident.
- Only one computer fails: local profile, cached credentials, device registration, or client state becomes more likely than a tenant-wide outage.
- Many users fail at once: check Microsoft 365 service health and tenant-wide Entra configuration before rebuilding individual profiles.
First response: restart Outlook after the service fix
Microsoft stated that the May 2025 service change fixed the incident and instructed customers to restart Outlook so the client could pick up the change. Close every Outlook window, confirm that Outlook is no longer running in the background, and start it again. On Windows, check Task Manager for any remaining OUTLOOK.EXE process before reopening the app. On Mac, quit Outlook from the menu or press Command-Q, then launch it again.
Restarting is useful only after the service or tenant state is corrected. Repeatedly entering the password will not repair a disabled service principal, a denied Conditional Access request, or a corrupted local token.
Administrator fix: enable the Microsoft Information Protection API
This change requires an administrator who can manage enterprise applications in Microsoft Entra ID. A normal Outlook user cannot enable a tenant application. Use the following path when the Entra sign-in failure identifies resource ID 40775b29-2688-46b6-a3b5-b256bd04df9f or when your tenant administrator confirms that the Microsoft Information Protection API application is disabled.

- Sign in to the Microsoft Entra admin center with an account that has sufficient application-management privileges. A Global Administrator can perform the check described by Microsoft.
- Open Enterprise applications.
- Search for Microsoft Information Protection API. If the name does not appear, search for the resource ID
40775b29-2688-46b6-a3b5-b256bd04df9f. - Open the matching enterprise application.
- Select Properties.
- Set Enabled for users to sign-in to Yes.
- Select Save.
- Ask an affected user to close and restart Outlook, then try the sign-in again.
Do not disable or recreate unrelated enterprise applications while looking for this setting. Verify the resource ID carefully: changing the wrong application can affect other Microsoft 365 workloads and will not address a 4usqa failure.
Why this setting matters
Enterprise applications represent service principals that allow a tenant to use a Microsoft service. If the relevant service principal is disabled, Entra ID can reject the token request with AADSTS500014 even though the user’s password is correct and the mailbox is licensed. Enabling user sign-in allows the application to participate in the authentication flow again; it does not bypass Conditional Access or grant a user a license.
Do these 3 things before closing this tab:

Use the following checks in order. Record what changes and which users are affected so the administrator does not erase evidence needed for diagnosis.
1. Check Conditional Access results
Conditional Access can block a desktop client because of location, device-compliance, authentication-strength, risk, or client-app conditions. Review policies that apply to the affected user, application, device platform, and sign-in risk. In the Entra sign-in log for a failed attempt, inspect the Conditional Access tab and note whether a policy reported Failure, Report-only, or a successful grant.
Do not disable a production policy broadly as a first step. If your organization permits testing, use a controlled pilot account or a temporary, narrowly scoped exclusion approved by your security team. Restore the original policy immediately after testing.

2. Confirm the Microsoft 365 license and service assignment
Verify that the user has an active Microsoft 365 or Office 365 license that includes Exchange Online and that the Exchange service plan is enabled. A user can authenticate successfully yet lack a mailbox service, while a blocked or expired license can produce a different failure later in the flow. Compare an affected account with a working account in the same tenant rather than assuming that an Outlook installation proves the user is licensed.
3. Sign out and refresh stale sessions
When web sign-in works but Outlook does not, stale refresh tokens or a changed device registration may be the cause. Sign out of Microsoft 365 sessions where your organization’s policy allows it, close Outlook, and restart the device. Reopen Outlook and authenticate with the intended work or school account. If the device was recently renamed, reimaged, joined, or removed from management, ask the device administrator to verify its Entra registration and compliance state.
4. Re-add or recreate the Outlook profile
Use profile recreation only after tenant settings, licensing, and sign-in logs have been checked. A profile reset affects local account configuration and cached data, so make sure any required local archives, signatures, and account settings are documented first. Add the account again using the organization’s normal Microsoft 365 sign-in flow. If a newly created profile fails in the same way on multiple machines, stop repeating local repairs and return to tenant-level investigation.
5. Review recent configuration changes
Ask whether anyone recently changed enterprise-application permissions, Conditional Access, security baselines, authentication methods, device-management rules, scripts, or service-principal settings. Automated scripts can disable an application or alter consent and permissions without an end user seeing the change. Compare audit records and configuration history with the time the first failures appeared.
Use Entra sign-in logs as the deciding evidence
Local Outlook messages are useful symptoms, but the Entra sign-in record usually identifies the blocked stage. An administrator should capture a failed event for the affected user and record:

Common mistakes that waste time
Treating 4usqa as a password error
A password reset can be appropriate when Entra explicitly reports invalid credentials, but 4usqa with CAA2000B or AADSTS500014 points first toward the authentication resource, policy, token, or client flow. Resetting a known-good password repeatedly can lock the account and obscure the original event.
Rebuilding every Outlook profile immediately
Profile recreation cannot re-enable a disabled service principal or change a tenant Conditional Access decision. If several users fail at the same time, collect tenant evidence before making local changes that create extra variables.
Assuming a working browser proves the desktop client is healthy
Browser and desktop sign-ins can use different client-app conditions, token caches, device signals, and application resources. A successful web session narrows the problem; it does not rule out a desktop-specific policy or stale registration.
Changing broad security policies without a test plan
Disabling Conditional Access or enterprise applications across a tenant can create a larger security incident. Use the narrowest approved test, document the original setting, and restore it after the result is known.

When to contact Microsoft support
Escalate through a Microsoft 365 Admin Center support request when the documented application is enabled, Outlook has been restarted, and the failure remains—or when the Entra portal is inaccessible to your administrators. Microsoft Q&A guidance directs a Global Administrator to submit the service request if the tenant team cannot resolve the issue.
Include the affected scope, Outlook edition and operating system, first-failure time, whether web Outlook works, the exact 4usqa/CAA2000B/AADSTS500014 text, the resource ID, relevant Conditional Access results, license status, and Entra sign-in-log identifiers. State whether the Microsoft Information Protection API enterprise application was already enabled or was changed during troubleshooting. This gives support a reproducible timeline without exposing credentials.
Current status of the May 2025 incident
Microsoft’s classic Outlook known-issues index lists “Errors CAA2000B and 4usqa when trying to sign in to classic Outlook” as FIXED under the May 2025 updates. Microsoft’s incident note says the service change was made on May 14, 2025 and that Outlook should be restarted to pick up the change.

That status applies to the documented incident, not to every future appearance of the tag. A new 4usqa message should be correlated with the current Entra sign-in record, tenant configuration, and Microsoft service-health information before an administrator assumes that the 2025 disabled-service-principal condition has returned.
