Painkiller returned as a new first-person shooter on October 21, 2025. The game is out now for PC, PlayStation 5, and Xbox Series X|S. Anshar Studios described it as “a modern reimagining” of the franchise; its design centers on fast movement, demon-slaying combat, and online co-op rather than a story-first campaign.
What is the new Painkiller?
The game puts players in Purgatory, where they are offered a chance at redemption: stop the fallen angel Azazel from invading Earth. Azazel’s monstrous children, the Nephilim, are among the enemies players face.

It is a first-person shooter built around jumping, dashing, and hooking through gothic environments while fighting demons and larger foes. The official game page describes its additional Rogue Angel mode as taking place in handcrafted randomized arenas.
How do combat and progression work?
Weapons and movement
Players carry up to three weapons: the always-equipped Painkiller and two additional weapons selected from six unlockables. Each weapon has two alternate-fire modes. Movement abilities—including jumping, dashing, and hooking—let players traverse arenas while engaging enemies.

Characters and Tarot cards
Players choose from four characters, each with different perks. Tarot cards provide ability upgrades; Saber Interactive Support lists 44 cards available at launch. These systems add character and build choices to the combat, though the official materials cited here do not establish how the new game’s systems compare feature by feature with the original.
Can you play solo or in co-op?
Online co-op supports up to three players, with matchmaking and crossplay. Solo play is also available offline, with AI-controlled teammates. There is no couch co-op, so players looking for local split-screen should not expect it.

Is it a story-focused campaign?
The Purgatory-and-redemption premise gives the action a narrative frame, but the official FAQ says the focus is co-op action. It notes that story threads are present without positioning the game as a narrative-first experience.

Physical editions and additional content
Saber Interactive’s FAQ says the base edition is available physically, but it does not identify retailers or confirm stock in any particular country. The FAQ also says there are no microtransactions; optional paid post-launch DLC is cosmetic and does not affect gameplay.
