Psychic damage in Baldur’s Gate 3 comes from spells and spell-like effects, actions and features, weapons, conditional equipment effects, and creature abilities. The BG3 Wiki’s Psychic damage category listed 88 pages when checked in October 2026, but that is a count of categorized wiki pages—not 88 attacks or options available to player characters. The categories below distinguish common player-facing sources from entries that may be creature-only, conditional, or unobtainable.
What counts as a psychic damage source?
Psychic is one of the game’s 13 damage types. A source can deal psychic damage directly, add a separate psychic damage roll to an attack, or change the damage type of damage that would otherwise be physical. That distinction matters when reading an item or ability description: a psychic die added on a trigger is not necessarily the same source as the weapon’s own damage, and a damage-type change is not simply another bonus die.
Spells and spell-like effects
Psychic damage entries include spells and spell-like effects such as Dissonant Whispers, Phantasmal Killer, Mind Spike, and Shadow Blade. These are examples from the wiki’s category, not a complete spell list. Actions and features
Indexed actions and features include Concentrated Blast, Mind Blast, Stage Fright, Psionic Overload, Shadow Strike, and the special Soulbreaker action. A category entry does not guarantee a player character can use it: some actions are tied to creatures or particular circumstances. Confirm the source’s availability and trigger on its own page or in its in-game description before building around it.
Weapons with psychic damage
The Psychic damage page identifies these weapons as dealing psychic damage as part of their regular attacks:
- Blade of Oppressed Souls
- Ketheric’s Warhammer
- Shadow Blade
- Sword of Screams
- Voss’ Silver Sword
Mind Sundering Dagger is also identified there, but marked unobtainable. It therefore belongs in an inventory of indexed sources, not a practical player build list. Availability can depend on game circumstances; check the current item description and acquisition details before relying on a weapon.

Equipment that adds psychic damage conditionally
Several equipment effects add psychic damage only when their stated condition is met. The trigger is part of the source, not an optional detail.

The category also includes creature abilities and condition-related entries. This makes it useful as a broad index, but not as a ready-made player inventory: an NPC or enemy ability may not be obtainable by a character, and a condition may deal damage only in a specific context. How resistance, vulnerability, and damage sources affect the result
- Resistance: halves damage of the relevant type.
- Vulnerability: doubles damage of the relevant type.
- Immunity: reduces damage of that type to zero.
If a target has both resistance and vulnerability to the same source, they cancel each other; neither cancels immunity. BG3 Wiki: Damage
Resonance Stone and psychic vulnerability
The Resonance Stone grants Steeped in Bliss to its wielder and those around them, making affected creatures vulnerable to psychic damage. That can increase psychic damage taken by allies as well as enemies, so positioning and party exposure matter. BG3’s damage rules generally apply modifiers once per source, and a single weapon attack can contain multiple damage types. A weapon’s base damage, an equipment bonus, and a separate effect should therefore be considered as distinct sources where the game treats them that way; simply adding all displayed dice together may not predict how modifiers apply. Shadow Blade illustrates a different interaction: some attack damage, including Sneak Attack, changes to psychic rather than adding a separate psychic roll. BG3 Wiki: Damage

Choosing a psychic-damage option
There is no verified DPS ranking established for these sources. To compare options for a build, assess the traits that determine whether they work in your situation:
- Damage structure: direct spell or action damage, a bonus attached to a weapon attack, or a damage-type change.
- Reliability: always available versus conditional on concentration, Advantage, Rage, ancestry, or a specific circumstance.
- Access: obtainable player equipment or abilities versus creature-only or unobtainable category entries.
- Cost to use: action economy and any concentration requirement.
- Target matchup: whether the target resists, is vulnerable to, or is immune to psychic damage.
