Spare her unless your roleplay calls for killing her or you specifically want the robe and accept losing Karlach.
What happens if you kill Karlach?
If you side with Wyll and kill Karlach in Act One, she is gone for the rest of the playthrough: she cannot join your party, and you lose access to her companion storyline. Mizora rewards Wyll with the Infernal Robe. It is a tangible item reward, but the choice does not make the robe essential to Wyll’s build.
This route can fit a character who obeys Wyll’s hunt or trusts the paladins’ account. Mechanically, though, the trade is a specific reward for losing a substantial companion and her story.
What happens if you spare Karlach?
Trusting Karlach’s account and resolving the Act One conflict peacefully lets her become a companion. Wyll faces consequences from Mizora for disobeying her, but sparing Karlach does not break his pact or end his larger story.

Keeping Karlach also preserves access to her later story and gives you more options for how her journey ends. The choice is not just dialogue flavor: killing her closes those options, while sparing her leaves them available.
Does saving Karlach mean she will die at the end?
No. Saving her in Act One does not determine her final outcome. Her infernal engine remains a major story problem, and the base game does not offer an ordinary permanent fix for it. Later circumstances and dialogue shape what happens.

Larian’s official Patch 2 notes describe an endgame choice involving Karlach and the Blade of Avernus: if Karlach is in the party and he offers to go with her, the player can go with them, go with Karlach alone, or let them go without the player. The character reference on bg3.wiki also describes circumstances in which she returns to Avernus with Wyll, the player character, or both. These are later possibilities, not guarantees created by sparing her.

- Choose to spare her if you want the most companion content and flexibility later.
- Choose to kill her if it suits your roleplay or you specifically value Wyll’s Infernal Robe more than keeping Karlach.
The decision is yours to make for the story you want to play. If you are uncertain, spare Karlach: it preserves more options, and it does not lock in her ending.
