Neither version of V is generally better for the campaign or combat. Choose the voice and presentation you want to play; if you have a specific major romance in mind, check its body-type and voice requirements before creating your character.
Does V’s gender choice change gameplay?
The evidence cited here does not establish a general advantage for a masculine or feminine V in combat, missions, or the main campaign. CD PROJEKT RED’s game overview describes skills, perks, weapons, hacking, implants, combat, and stealth without assigning those options by gender: Cyberpunk 2077 official game page. Build for the playstyle you want rather than expecting a gender-specific gameplay bonus.
That does not prove every line of dialogue or story interaction is identical between presentations. The available sources do not provide a controlled comparison of dialogue, mission outcomes, or playtime, so claims that the campaign is perfectly the same—or that one choice unlocks a broadly superior story—are not established.
How character creation works
Cyberpunk 2077 lets you customize V’s body and select a voice; those choices are not simply one inseparable male-or-female package. In 2019, CD PROJEKT RED senior concept artist Marthe Jonkers described the design intent as choosing a body type and mixing it with one of two voices, rather than choosing a gender label: Steam News interview excerpt, August 28, 2019. That statement explains the character-creation approach; romance guides are the more useful source for current relationship requirements.

Which V presentation is needed for each major romance?
The four major relationships covered by current guides use body type, and in two cases voice, to determine eligibility. “Masculine” and “feminine” below refer to the game’s presentation requirements, not a claim about a player’s identity.

These requirements are reported by Push Square’s romance guide (June 4, 2025) and the RPG Site romance guide. RPG Site distinguishes short flings from the four major romances and says those brief encounters are open regardless of body type and voice.

Plan around the relationship you want
- Pick the major romance that matters most to you.
- Match V’s body type to that character’s requirement.
- If the guide specifies a voice as well, select the matching voice during character creation.
Romance can also change interactions in some endings or epilogue scenes, so it can shape parts of the story beyond the relationship itself. The romance guides describe those effects; they do not mean that one V presentation is best for every ending.
What about the voice actors?
Which voice performance feels right is subjective. The cited sources do not establish an objective ranking, so sample both if voice is the deciding factor and choose the one that suits your version of V.

A simple decision rule
- No romance preference: choose the voice and character presentation you are most interested in role-playing.
- One specific major romance: use its body-type and voice requirements to make your choice.
- Mostly interested in combat or builds: choose based on preference; the official game overview does not identify a gender-based combat advantage.
CD PROJEKT RED’s 2018 announcement said character choices such as gender, appearance, class, and background could influence the game’s shape, but it describes early plans rather than proving present-day gender mechanics: CD PROJEKT RED announcement, June 12, 2018. For romance interactions, the studio’s lead quest designer Paweł Sasko told GameSpot that an NPC’s sexual orientation also plays a role: GameSpot interview, August 30, 2019.
