When Skalpel climbs onto the coffin, players recommend attacking the coffin to interrupt his powered-up sequence. Treat this as community advice, not a guaranteed mechanic: players also report that getting close enough to do it safely can be difficult. If the sequence continues, shift your focus to surviving the plague pressure and taking clear openings to punish him.
Disrupt the coffin sequence if you can
Several players report that hitting the coffin while Skalpel is on it can interrupt or prevent his powered-up state. The advice is worth trying when the opportunity appears, but it is not an officially documented guarantee. Do not trade a large amount of health just to reach the coffin; if the attempt is unsafe or fails, return to defense and look for a recovery window.
PowerPyx lists Skalpel as boss 12 in its guide to the game’s 16 main bosses. Its video guide is described as using Normal difficulty with no Spirits of Advocacy. Survive the later phase, then punish safely
Players describe plague pressure and a more difficult survival phase later in the fight. Prioritize avoiding damage and plague buildup over forcing attacks. When Skalpel finishes an attack or otherwise leaves a clear recovery, use that opening for a controlled punish, then be ready to defend again.

Some players say Skalpel’s health falls over time, but reports differ on whether waiting is practical. Do not rely on passive damage as a guaranteed way to win: stay alive, evade the pressure, and take safe opportunities to attack.
Choose your defense and preparation
Khazan’s combat rewards recognizing which attacks are better dodged and which can be blocked; a single defensive response is not right for every move. PowerPyx favors the spear in its general build discussion but presents weapon choice as personal, not a requirement for this encounter. Use the weapon and defensive approach you can execute reliably rather than rebuilding around an alleged Skalpel weakness.

If plague or elemental pressure is causing trouble, consider using available in-game status cleanses or elemental-resistance consumables. Weapon enhancers are another in-game preparation option. These can support a safer attempt, but the available sources do not establish a definitive weakness or a required item for Skalpel.
