Spawn: The Eternal follows Al Simmons, a government assassin who is murdered, bargains with the demon Malebolgia in Hell, and returns to Earth as a scarred Hellspawn. He accepts the deal because he wants to see his beloved former wife, Wanda, but the price is service in Hell’s army. The game’s central conflict is whether Spawn can use the powers forced on him while refusing to remain Malebolgia’s weapon.
This guide separates the PlayStation game’s manual-based story from the wider Spawn mythology, then identifies the people, demons, angels and human allies a new player is most likely to encounter.

What Spawn: The Eternal is about
The game begins after Al Simmons has already died and been resurrected. In life, Simmons was a highly trained government assassin. He eventually questioned the purpose of the missions he was ordered to carry out, and an associate arranged for him to be burned to death. His death does not end his story. In Hell, Simmons negotiates with Malebolgia, ruler of Hell’s Eighth Sphere, for one chance to return and see Wanda.

Simmons comes back as a Hellspawn, a soldier intended for Hell’s coming Armageddon. He is no longer fully the man he was: his body is scarred, his memories are damaged, and his new abilities come from the same infernal power structure that claims ownership of him. The living costume, supernatural strength and other powers make him formidable, but they also mark the debt he owes.

The immediate game story follows Spawn as he moves through three realms at different points in time. He fights creatures sent by Malebolgia, searches for information that may lead back to Hell and tries to understand the forces controlling his resurrection. The eventual objective is more than survival. Spawn seeks a way to confront Malebolgia and reclaim the freedom that the bargain was designed to take away.
How Al Simmons became Spawn
Al Simmons before the resurrection
Al Simmons starts as a professional killer working for the government. His training explains why Spawn is dangerous even before supernatural powers are considered: he understands weapons, tactics and pursuit. The important change in his life is moral rather than physical. Simmons begins to question the missions he performs and the people who benefit from them. That doubt places him in conflict with the organization around him and contributes to his betrayal.
An associate has Simmons burned to death. The murder is the hinge between the human story and the supernatural one. Simmons is not summoned to Hell because he is a simple monster; he arrives there carrying regret, anger and an unresolved attachment to Wanda.
The bargain with Malebolgia
Malebolgia offers Simmons a transaction rather than a rescue. Simmons wants to see Wanda again, and Malebolgia controls the means to return him to Earth. The game manual states the premise plainly: “You struck a deal with Malebolgia, ruler of the Eighth Sphere of Hell.” The wording matters because Spawn’s powers are not a free gift. They are the consequence of a contract made under extreme emotional pressure.
Malebolgia expects Simmons to become one of Hell’s soldiers. The bargain therefore has two levels. Personally, it gives Simmons the possibility of seeing Wanda. Politically, it adds a powerful fighter to Hell’s forces as Armageddon approaches. Spawn’s struggle begins when those two purposes separate: he wants the person he loves, but he does not want to carry out the will of the demon who restored him.

The transformation into a Hellspawn
Resurrection changes Simmons’s identity as well as his body. The manual asks, “Are you Al Simmons now? No, you are one of the Hellspawn.” That is not merely a dramatic label. It describes the status Hell assigns him and the way its rulers expect him to behave. His costume is alive, his powers are supernatural and his appearance bears the scars of death. Missing memories reinforce the sense that his return has extracted a permanent cost.
Spawn’s resistance is therefore an argument about agency. He cannot undo the fact that he is a Hellspawn, but he can decide whether that status determines every action he takes. The game’s enemies and objectives repeatedly place him between obedience and self-determination.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsWhy Wanda is the emotional center
Wanda Blake is Al’s beloved former wife and the reason he accepts Malebolgia’s offer. A new player can understand the entire origin by keeping that motivation in view: Simmons does not enter the bargain because he wants to rule Hell or gain a costume. He wants one human connection that death has taken from him.
Wanda’s life has continued while Al has been dead. She is married to Terry Fitzgerald and has a daughter, Cyan. That fact makes Spawn’s return painful rather than triumphant. Seeing Wanda does not restore the old life, and his presence threatens a family that formed in his absence. The emotional stakes are consequently not a straightforward rescue plot. Spawn must face what he lost, what Wanda has built without him and whether his desire to return can coexist with her safety.
Wanda also connects the game’s supernatural conflict to ordinary human consequences. Malebolgia treats Simmons as military property, while Simmons thinks in terms of a wife, a family and a life he can no longer simply resume. The tension between those viewpoints gives the game’s battles a personal purpose.

What Malebolgia wants from Spawn
Malebolgia is the demon ruler who resurrects Simmons and expects him to serve Hell. In the game’s premise, he is not just a final boss waiting at the end of a level. He is the authority behind the contract, the source of the military role imposed on Spawn and the force sending creatures to obstruct or control him.
Hell’s plan is to use Spawn in the coming Armageddon. That explains why Malebolgia tolerates the resurrection at all: Simmons is valuable as a fighter. Spawn’s growing refusal to accept orders turns the bargain into a power struggle. Every hostile creature and infernal command can be read as an attempt to remind him that his abilities came from Hell and that Hell expects repayment.
The game eventually allows Spawn to seek Malebolgia directly. Reaching that confrontation is both a practical objective and a declaration that the contract does not define his future. The story does not require a new player to know every later comic development to understand the conflict: one side wants a soldier, and the soldier is trying to become his own person.
How the game’s story unfolds
Travel through three realms
The manual-based account sends the player through three realms at different points in time. The realms are part of the game’s structure, not a single continuous neighborhood. Their changing settings and time periods reinforce the idea that Spawn is moving through a larger supernatural conflict rather than simply clearing rooms in one location.
The game does not present the journey as a history lecture. Players learn through movement, combat and discoveries. Hostile creatures show what kinds of forces Malebolgia can deploy, while environmental secrets point toward the relationship between the realms and Hell.

Fight creatures sent by Hell
Combat has a narrative function. Spawn is attacked because he is useful to Hell but increasingly unreliable. The enemies he encounters are evidence that the infernal hierarchy is monitoring his actions and trying to keep him on its intended path. Defeating them gives Spawn room to continue investigating, but it does not erase the underlying bargain.
Uncover secrets and look for a route back to Hell
The player’s progress depends on learning more about the forces surrounding Spawn. Secrets may reveal a path back to Hell or clarify why the creatures are being sent against him. This investigative element is important for newcomers: the objective is not simply to reach the next arena. Spawn is trying to understand the system that created him well enough to challenge it.
Confront Malebolgia
The story builds toward an attack on Malebolgia in search of freedom. Whether a player approaches the confrontation as revenge, escape or moral defiance, the practical goal is the same: stop being an instrument controlled by the demon who made the deal. The game’s arc turns the classic descent-to-Hell premise inside out. Spawn is already Hell’s creation; his adventure is about refusing to remain Hell’s property.

Characters new players should understand first
Violator: surveillance made personal
Violator is assigned by Hell to keep tabs on Spawn. His importance is not only that he is a demon opponent. He makes the infernal chain of command visible. Spawn may look independent on Earth, but Violator’s presence reminds him that Hell considers his resurrection an assignment with supervision.

Cogliostro: a guide to the larger mythology
Cogliostro is an older, mysterious figure who serves as Spawn’s tutor or guide in the wider mythology. He is useful for readers who find the setting’s rules confusing because he provides context rather than representing a simple faction. His role is best understood as instructional and interpretive: he helps Spawn—and the audience—make sense of what a Hellspawn is and what choices remain possible.

Terry Fitzgerald and Cyan: the life Al missed
Terry and Cyan are not interchangeable with Wanda. Terry is the man who shares Wanda’s present life, while Cyan is their daughter. Together they show that Al’s death created consequences beyond his own suffering. Spawn’s old identity remains emotionally real, but the family he remembers has changed. This is why returning to Wanda cannot be treated as a reset button.
Sam Burke and Twitch Williams: human perspective
Sam Burke and Twitch Williams are police detectives and friends of Spawn. Their investigative, human viewpoint contrasts with the grand claims of Hell, Heaven and Armageddon. They help anchor the story in recognizable concerns—evidence, danger, trust and protection—even when the threats are supernatural.
Game story versus the wider Spawn mythology
Spawn: The Eternal uses the core origin shared by the broader franchise: Al Simmons dies, makes a pact with Malebolgia and returns as Spawn. The emotional foundations—Wanda, the cost of resurrection and resistance to Hell—also carry across versions. Image Comics’ Spawn #118 explicitly connects Spawn’s present identity with Al Simmons and his original pact.
The game should still be treated as its own continuity. Its manual specifically describes travel through three realms at different points in time, creatures sent by Malebolgia, discoveries leading toward Hell and a possible attack on Malebolgia. Those are the game’s immediate events. The comics add later characters, reversals and developments that do not automatically occur during the PlayStation adventure.
A reliable way to avoid confusion is to label information by source. If a detail concerns the three realms, the player’s route or the game’s objective, it belongs to Spawn: The Eternal. If it concerns Angela, Mammon, Redeemer or later changes in relationships, identify it as part of the wider Spawn mythology unless the game itself establishes it.
Five things to remember while playing
- Spawn is Al Simmons after death. He is not an unrelated hero who happens to share a name; the resurrection transforms Simmons into a Hellspawn.
- Wanda motivates the bargain. His wish to see her again explains why he accepts Malebolgia’s offer.
- Malebolgia collects the debt. The demon’s resurrection comes with an expectation that Spawn will serve in Hell’s army.
- The costume and powers have a cost. They are part of the Hellspawn transformation, while scars and missing memories show what resurrection has taken from Simmons.
- The adventure is about resistance as much as combat. Spawn moves through hostile realms, learns the truth and seeks a way to escape Malebolgia’s control.
With those points in place, the game’s story is easier to follow. Each realm, enemy and revelation contributes to the same question: can a man who was resurrected as Hell’s soldier decide for himself what he will become?
