Full quest spoilers, including the killer’s identity and the Nathaniel branch.
To uncover the real killer in Carnal Sins, let Reverend Nathaniel Pastodi explain himself at Crippled Kate’s, then ask who left the letter on Patricia Vegelbud’s body. His answer points to coroner Hubert Rejk. If you attack Nathaniel before he gives you that lead, the quest can still end, but you lose the direct route to solving the murders. Complete the optional quest before starting The Isle of Mists, or it can fail.
How to start Carnal Sins
Carnal Sins follows the optional secondary quest Cabaret. During the cabaret opening, Dandelion learns that Priscilla has been attacked. Go with him to Vilmerius Hospital and speak to him and surgeon Joachim von Gratz. Von Gratz expects Priscilla to survive and connects the attack with a similarly mutilated corpse at the morgue. Agree to investigate.
Prima Games suggests level 16, Axii Delusion rank 3, and 200 crowns as preparation. These are guide recommendations, not stated entry requirements. The 200 crowns or Axii check also comes up later when dealing with Eustace, the corpse collector; that check is separate from the rank 3 Axii option for Nathaniel.

Investigate the murders
Examine Fabian Meyer at the mortuary
Meet von Gratz and follow him through the sewers to the Novigrad mortuary. Use Witcher Senses to find the dwarven woodcarver Fabian Meyer. His eyes have been replaced with burning coals, his throat treated with formaldehyde, and his heart removed and replaced with a scorched salamander egg. Coroner Hubert Rejk arrives during the examination. Von Gratz remarks that Hubert seems not to have aged in decades—a clue worth remembering.
Complete the three leads in any order
The investigation has three leads. Witcher.tools lists 200 XP for completing all three:
- Priscilla’s attack alley: Use Witcher Senses to find a knocked-over brazier and the smell of formaldehyde. The trail ends at an unusually high wall, suggesting the attacker made an inhuman leap.
- Fabian’s workshop and death scene: Examine the workshop and the place where Fabian’s body was posed among sculptures made by his brother, Gus. Fabian has far less blood than expected.
- Eustace: The corpse collector admits to robbing Fabian’s body but initially refuses to cooperate. The walkthrough lists two ways to get his note: Axii Delusion rank 2 or paying 200 crowns. The note, written on human skin, names Priscilla as the next victim.
The lack of blood, the leap, and Hubert’s unchanged appearance all point beyond an ordinary human killer.

Follow the case to Patricia Vegelbud
Hubert brings news of another victim, theology lecturer Joris Aquinus, and the investigation turns to Patricia Vegelbud. Go to the Vegelbud residence, follow Ingrid to Patricia’s room, and pursue the fleeing killer. Guards may stop Geralt during the chase; Ingrid clears up the misunderstanding. The letter found on Patricia directs Geralt to Sweet Nettie at Crippled Kate’s.

Find Nettie upstairs. Reverend Nathaniel Pastodi is torturing her, so he appears to be the killer. Do not attack him before he has explained himself if you want to identify the real murderer. Keep the conversation going and ask specifically who left the letter on Patricia’s body. Nathaniel says Hubert was the only person who knew where he would be and points Geralt to warehouse twelve, where Hubert is buying formaldehyde.
Some dialogue choices can lead to Nathaniel’s death before Geralt gets this information. If you kill him too early, the quest can still progress to a failure-style resolution, and a later confession note can reveal the truth, but you miss the direct lead to Hubert.

Choose Nathaniel’s outcome after he names Hubert
- Use Axii Delusion rank 3: Geralt can make Nathaniel stop torturing Nettie and leave. The walkthrough says Nathaniel remains unharmed.
- Kill Nathaniel: The walkthrough lists 100 XP, Tracker’s trousers, and the Ard Skellig aketon diagram for this outcome.
- Leave Nathaniel with Nettie: Nathaniel claims he will cause no permanent harm. This keeps the Hubert lead if he has already disclosed it, but leaves Nettie with him for the moment.
Find Hubert at warehouse twelve
Go to the warehouse near Novigrad’s docks. Hubert reveals that he is the murderer and a vampire. Defeat him to resolve the investigation. Prima Games describes Hubert transforming into a vampire during the fight; Witcher.tools lists 500 XP, the Assassin’s boots diagram, and a Lesser Svarog runestone for killing him.
Return to the hospital and finish the quest
Return to Vilmerius Hospital and speak with Dandelion about Priscilla. On the successful route, she is recovering and can speak again. Carnal Sins is optional, but it can be failed by advancing to The Isle of Mists before finishing it.

Witcher.tools lists the following XP and items. Item rewards are tied to the relevant branch; do not assume a single playthrough grants every item.

Best choice if you want to solve the mystery
Question Nathaniel fully and ask who left the letter before deciding his fate. Then follow his lead to Hubert and defeat him. If you want to spare Nathaniel, the source-supported option is Axii Delusion rank 3 after he has identified Hubert. Killing Nathaniel immediately does not necessarily stop the quest, but it removes the direct route to the real killer.
