Is Total Chaos a DOOM II mod or a standalone game?
It is both in its history and distinct in its present form. Developer Sam Prebble’s original project was an award-winning DOOM II total conversion. The 2025 release was rebuilt as a standalone game for PC and consoles, rather than being a mod that still requires DOOM II. The launch announcement describes a nine-chapter experience and identifies Fort Oasis as its setting.

What kind of game is Total Chaos?
It is a first-person psychological survival-horror game set on the isolated island of Fort Oasis. You explore ruins, face grotesque enemies and scavenge materials while deciding how to use limited supplies. Combat is part of the experience, but so are crafting makeshift weapons, upgrading equipment and managing what you carry.
The Steam listing identifies mental-health struggles and self-harm as themes, and warns of frequent violent combat against disfigured monsters. Those content details may help players decide whether the game is a good fit for them.
What has changed since launch?
New Game+ adds a new threat and story content
The official New Game+ update remixes the first seven chapters and introduces The Hunter, a persistent stalker enemy. It also adds Chapter 8, “Reckoning,” further narrative material, a Retro Tyler voice option drawn from the original mod, and additional save slots. See the official New Game+ announcement.
Retro Mode changes the presentation
A later update adds Retro Mode, with pixelated post-processing, lower-resolution textures and low-poly monsters inspired by the original DOOM II WAD. The same update includes a New Game+ epilogue, a map with location markers, save-time tracking, a garbage chute and a reworked Protector encounter.
