Choose the right portrait workflow

Import a custom portrait for your player character

Open this folder in Windows File Explorer:
%userprofile%AppDataLocalLowOwlcat GamesWarhammer 40000 Rogue TraderPortraits. If the Portraits folder is missing, create it. -
Create a separate subfolder for each portrait set, using a name that helps you identify it.

Put three PNG files in that subfolder. The Rogue Trader Wiki guide lists these filenames and dimensions:

Fulllength.png, exactly as shown. These sizes come from the third-party guide, not an official Owlcat specification. If the game’s sample or template files are available, use their dimensions as your reference. -
Start the game and look for the portrait set in the custom portrait choices during character creation. If it does not appear, check the folder location, subfolder, filenames, PNG format, and dimensions.
The guide also mentions Notra and Rogue Trader Portrait Manager as optional tools for preparing portrait images; neither is required to follow the folder method. It notes that sample or template files may be available. [Rogue Trader Wiki guide]
Change a portrait after starting a campaign
A Steam community answer describes changing an existing character’s portrait with ToyBox: open its Party tab, use the character’s Stats interface to select a custom portrait, save, and restart the game. This is a community-reported, mod-dependent method rather than an official current procedure; exact labels and behavior may vary by ToyBox and game version. [Steam community discussion]

Replace NPC or companion portraits
Importing a player portrait does not by itself replace NPC or companion portraits. The Custom NPC Portraits mod describes a separate layout: folders for NPCs and Companions, with character-name subfolders containing Small.png, Medium.png, and Fulllength.png. Its instructions also call for enabling the mod and refreshing portraits. Follow that mod’s own current page for exact setup, since the workflow is specific to its releases. Why portrait dimensions may differ between guides
The older Steam discussion gives a different set of dimensions: 692 × 1024, 330 × 432, and 185 × 252 pixels. The source does not establish which game version or setup those figures apply to, so do not mix them with the Wiki guide’s dimensions.
